The contract specifies the procurement of a pediatric electrode pad compatible with the Cardiac Science AED G3 Pro defibrillator, identified by NSN 6515-01-558-6948 and manufacturer part number 9730-002. Each unit must have a non-extendable 24-month shelf life, with no more than four months elapsed between manufacture and government delivery, and must be clearly marked with the lot number and date of manufacture prefaced by "MFD". Packaging must comply with DLA’s Packaging Requirements and Medical Marking Standard No. 1, superseding MIL-STD-129, and must ensure protection from damage while enabling safe, low-cost delivery via common carrier to the destination at Fort Hood, Texas. The item is regulated by the FDA, and bidders must explicitly state the source and part number being offered. All packaging and labeling must adhere to the DLA Master List of Technical and Quality Requirements, which take precedence over ASTM D3951, and palletization must follow DLA guidelines. The unit of issue is each, with a single unit quantity required, and delivery is mandated within 20 days FOB destination. Shipping must be traceable, excluding parcel post, and all documentation must be properly marked with government identifiers and procurement references.
