Amendment 3: Closing Date Extension - Phoenix Area IHS Dental Equipment PM and Repair Services IDIQ contract

The contract is an Indefinite Delivery/Indefinite Quantity (IDIQ) Firm Fixed-Price agreement for preventive maintenance and repair services for dental equipment across 12 Indian Health Service facilities in Arizona, with potential performance extending from June 1, 2026, through May 31, 2031, inclusive of one base year and four one-year option periods. The contractor is required to perform all scheduled and unscheduled maintenance, repairs, and inspections in strict compliance with Original Equipment Manufacturer specifications and industry standards including ISO 13485:2016, ANSI/AAMI EQ56:2013, NFPA 99, and The Joint Commission’s EC.02.04.01 and EC.02.04.03, while adhering to OSHA, HIPAA, and applicable federal, tribal, and local regulations. All calibrations must be traceable to NIST, and services must be documented with detailed service reports, equipment spreadsheets, and calibration certificates. Equipment must be labeled with asset tag numbers, technician identifiers, service dates, and next PM dates using durable, laminated labels that remain legible in extreme temperatures, with all prior labels removed before application. Failure to meet standards requires immediate correction at no additional cost. The contractor must maintain active ISO 13485:2016 certification and assign qualified personnel, including a program manager as the primary point of contact, with all technicians required to hold appropriate certifications and be available to respond to emergencies within 72 hours during business hours.
