Sustainability planning involves assessing internal capacities, building partnerships, diversifying revenue sources, and creating measurable outcomes to demonstrate impact to funders. The work is critical to maintaining access to care for vulnerable populations and must be executed with precision to meet the agency’s mission of supporting community living and independence for older adults and individuals with disabilities.
