Professional Education, Promotion Activities, and Evaluation Support to Prevent and Manage Chronic Diseases

The New Hampshire Department of Health and Human Services, Division of Public Health, is soliciting proposals for professional education, promotion, and evaluation support to manage chronic disease, screening, and preventative health programs. The selected vendor will support program areas including asthma, diabetes, arthritis, cancer, and cardiovascular health by coordinating partner engagement, assisting with evidence-based interventions, and managing administrative and communication functions. A primary requirement is the outreach, administration, and monitoring of at least 20 innovative pilot projects, with allocated funding of 500,000 dollars for State Fiscal Year 2027 and 250,000 dollars for State Fiscal Year 2028. Award selection is based on a point system where the technical response accounts for 700 points and the cost proposal for 300 points, emphasizing that this is not a low-cost award. Vendors must meet a minimum technical threshold of 400 points to be considered. Compliance with HIPAA, the HITECH Act, and various federal and state non-discrimination and drug-free workplace laws is mandatory.
