Professional Education, Promotion Activities, and Evaluation Support to Prevent and Manage Chronic Diseases

The New Hampshire Department of Health and Human Services, Division of Public Health (“Department”) is seeking responses to this Request for Proposals ("Solicitation" or "RFP") from qualified Vendors to support the Department’s chronic disease, screening, and preventative health programs. The selected Vendor will provide support across program areas by coordinating partner engagement, assisting with evidence-based intervention activities, and supporting evaluation, training, administrative, and communication functions as directed by the Department.
