This RFQ is being issued as Firm Fixed Price, UNRESTRICTED. All responsible sources may submit a quotation which shall be considered by the agency. Non-Personal Maintenance Service for the Fume Hoods and Bio-logical Cabinets located at Evans Army Community Hospital, Fort Carson, CO , FOB Destination. These items/supplies are for use at Evans Army Community Hospital, Fort Carson, CO and the proposed contract is for a base period of one year plus option periods. All Contractors shall provide an offer for the following CLINs and option line items, CLIN 0001, The contractor shall provide a Non-Personal Maintenance Service for the Fume Hoods and Bio-logical Cabinets located at Evans Army Community Hospital, Fort Carson, CO which include ALL labor, travel, material, equipment and parts (excluding consumables) necessary to perform preventive maintenance, and annual certification. The contractor will provide auxiliary support services with a dedicated support team, all required parts and labor/travel (when necessary) to maintain the equipment in a serviceable condition in accordance with the manufacturer's specifications, priority phone support and continuous product information updates and bulletins. Contractor will provide technical assistance and replacement parts. Contractor service technician will continue all services until the instrument is 100% operational. Equipment must be restored to a serviceable condition within twelve (12) hours after arrival at the work site. Period of Performance: 1 Oct 2011 thru 30 Sep 2012 (Subject to the Availability of FY12 funding.)
