NOTICE OF INTENT TO AWARD SOLE SOURCE.  THIS IS NOT A REQUEST FOR COMPETITIVE QUOTES. This is a notice of intent to solicit and award a sole source contract set forth in accordance with the Revolutionary FAR Overhaul (RFO) 12.102(a), Restricting competition.


The Department of Veteran Affairs, Veterans Health Administration, Network Contracting Office 12 (NCO-12) Great Lakes Acquisition Center, intends to negotiate a sole source Firm-Fixed Price to IBA-Dosimetry USA Inc: 1510 Woodcock Street, Unit 23, London, Ontario, Canada IAW Federal Acquisition Regulation/Revolutionary FAR Overhaul 12.102. This is for procurement of the Quasar MRID3D (5.0 mm) MR Image Distortion System for the Radiation Oncology Department located at Clement J. Zablocki Veterans Affairs Medical Center: 5000 West National Avenue, Milwaukee, WI 53295-0001. The North American Industry Classification System (NAICS) for this requirement is: 334510 - Electromedical & Electrotherapeutic Apps Mfg. OEM, Modus Medical Devices dba IBA Dosimetry America Inc is the sole manufacturer and the sole distributor for this requested end-product. The facility would like to obtain this order asap.
