The Ohio Department Behavioral Health – Ohio Pharmacy Services is looking to purchase a specific dialysis product for the treatment chronic kidney disease. The requested dialysis product must be specific to the needs of distribution. The specific dialysis product is listed below. Annual usage is included, but subject to change. This product will be purchased monthly by OPS from awarded bidder. Substitutes for this item may be submitted but should resemble the product listed as close as possible and will be subject to review. Should the substitute not meet the needs as determined by our facilities, they will be disqualified. Bids with the preferred item will receive first consideration. The Ohio Department of Behavioral Health (DBH) will award this project to the lowest-cost responsive and responsible supplier. The awarded supplier must include all applicable costs for delivery, setup, and warranty in their submission.Substitutions are not permitted unless explicitly authorized within the solicitation documentation.All pricing must be clearly entered into the OhioBuys item grid. If a separate quote is submitted, the total must exactly match the amount listed in the item grid.
