**THIS IS AN INDIAN-OWNED SMALL BUSINESS ECONOMIC ENTERPRISE (ISBEE) SET-ASIDE**


The Indian Health Center, Kayenta Health Center located at  Highway 160 Mile Post 394.3, Kayenta AZ 86033 has a requirement for HuFriedy brand restorative dental instruments and sterilization accessories to support clinical operations and standardize infection control protocols. 


The Government will award the contract to the responsible offeror whose quote is determined to be the Lowest Price Technically Acceptable (LPTA). Under this source selection process, the award will be made to the vendor that offers the lowest total evaluated price and is deemed 'Acceptable' across all technical evaluation factors detailed in Section E of the solicitation document.


Quotation Submission Requirements
Please ensure all quotes are prepared in accordance with the following instructions. Failure to provide the required documentation may result in the quotation being considered incomplete for evaluation purposes.


1. Price Quote (Required – Standalone Document): The offeror shall provide a separate, standalone price quote document. The use of government forms (e.g., SF 1449) in lieu of a quote is not acceptable. The price quote must be presented in a clear, itemized format suitable for evaluation and must clearly include the item name/description, quantity, unit price, and total amount.


2. Technical Acceptability Information: The offeror shall provide a separate document, or include within the quote, a narrative and supporting documentation addressing the technical requirements outlined in Section E. This includes being an authorized reseller.


3. Submission Format: All required documents shall be submitted as separate attachments or combined into a single package. The Government will evaluate quotations based on the information provided in these documents. 


The Government shall keep the solicitation posted and open for quotes until June 1, 2026 12:00pm EDT
