Title: Prescription and Non-Prescription Safety Glasses at the Pentagon Location: Pentagon Administrative Information: THIS IS NOT A REQUEST FOR PROPOSAL OR QUOTE. This sources sought is issued solely for information and planning purposes and does not constitute a solicitation. Responses to this notice will not be returned. Vendors will not be notified of the result of this review. It is the intent of Washington Headquarters Services Acquisition Directorate (WHS/AD) to use this sources sought for market research purposes only. WHS/AD anticipates soliciting for award on General Services Administration eBuy (GSA eBuy). The Government shall not reimburse the costs of preparing responses to this request for information. No proprietary, classified, confidential, or sensitive information should be included in your response to this sources sought notice. The Government reserves the right to use any information provided by respondents for any purpose deemed necessary and legally appropriate, including using technical information provided by respondents in any resultant solicitation. The Department of Defense (DoD), WHS/AD is performing market research to determine the availability, interest, and capability of potential vendors under NAICS 446130 Optical Goods Store; FSC/SVC Code: 8415 and the availability of vendors with a General Services Administration Federal Supply Supplier (GSA FSS) contract. Background: WHS/AD anticipates establishing a Blanket Purchase Agreement (BPA), which will provide a selection of Occupational Safety and Health Administration (OSHA) and American National Standards Institute (ANSI) approved safety eyeglass frames and lenses, including both plastic frames and metal frames with adjustable nose pads. Frames must be standard and issued in suitable eye sizes to fit small, medium, and large people. Employees will try on frames and frames will be fit and adjusted for each employee's needs after the lenses are made. The contractor will take all required measurements for multi-vision employees, including pupil distance (PD), and multi-focal height. Technical Requirements: The Contractor shall perform in accordance with the following technical requirements: The contractor should expect to accommodate needs for no prescription, single, bifocal, and multi-focal vision, as well as progressive (no-line) lenses. The contractor will use OSHA approved lenses for safety glasses. Employees shall provide the contractor with a current vision prescription as well as voucher from Federal Services Directorate (FSD) management showing proof of authority to use the contractor's services. The contractor shall not accept work from any employee without both a valid vision prescription and written FSD voucher authorization. The contractor shall deliver the glasses when ready and dispense them at the Pentagon. The contractor shall provide fitting and adjustments of the frames for each employee upon delivery. The contractor shall provide these services and supplies within 10 days or less. There are approximately 200 employees needing glasses. The contractor shall be within close proximity of 60 miles of the Pentagon. Objective: The BPA encompasses all activities associated with the Operation of Optical Goods Store at the Pentagon. Task: A monthly pick-up, delivery, and adjustments from the Pentagon. Instructions: All interested and qualified vendors with the necessary capability to deliver the requirements may submit capability statements to the following Contracting Staff: lynette.m.thibodeaux.civ@mail.mil stephenie.w.charlot.civ@mail.mil Capability statements and suggested types of lens coverings are due by 10:00 AM (EST) on 4 NOVEMBER 2021. No phone calls will be accepted. Please respond to the following points in your capability statements: Company Name and Address. DUNS, CAGE Code and GSA Schedule Number. Company Point of Contact, Phone number, and email. Recommended/suggested NAICS codes for this effort. NAICS Codes associated with the Company. Identify contracts your company has been awarded under NAICS 446130 Optical Goods Store. Specific examples of prior efforts similar to the requirement; please include contract type (FFP, CPFF, etc.), contract award amount, contract number, Period of Performance, and a Government Point of Contact and email address for the Point of Contact. We are especially interested in prior experience in operating an Optical Goods Store. Business Size and Classification (Small Business, Veteran-Owed (VO), Service Disabled Veteran-Owned (SDVOSB), Woman-Owned (WO), HUBZone, or any of the other small businesses classifications). Description of your company's capability to provide the above mentioned technical requirement. Vendors shall provide input and best practices with regards to the contract type and the services - Optical Goods Store. Additional information requested: different types of lenses coating suggested and a price range for each type. Requirement: To qualify for submitting a capability statement in response to this Sources Sought Notice, all vendors must be: Registered in System for Award Management (SAM) (www.sam.gov) for NAICS 446130 Optical Goods Store. Small businesses only. Local to the Pentagon (District of Columbia, Maryland, and Virginia only - within close proximity of 60 miles of the Pentagon). Have a past performance of similar type of work within the last three (3) years. The contract/orders must have at least six (6) months of performance as of October 31, 2021.
