Chronic Disease Division (CDD) seeks a contractor to coordinate and implement 20 Living Well DC workshops through the Self-Management Resource Center (SMRC) for District residents with chronic diseases. The SMRC programs help people and their caregivers manage their symptoms, improve quality of life, and reduce healthcare costs. The SMRC program includes the following Living Well DC workshops; Chronic Disease Self Management Program (CDSMP), Diabetes Self-Management Program (DSMP), Cancer Thriving and Surviving (CTS), and Tomando Control de su Salud. Implementing these Living Well DC workshops in community settings such as senior centers, residential facilities, and community-based organizations (CBOs) with an emphasis on locations with the highest needs in Wards 5, 7 and 8, will help CDD achieve its mission by providing District residents struggling with chronic disease with the necessary information and skills to manage their conditions.
