SLCo HEA131931-1 Lease Health Clinic Space in the Salt Lake Public Health Center (City Clinic) RE-ISSUE - RFP

Salt Lake County, on behalf of its Department of Health, is re-issuing a solicitation for the lease of 4,154 square feet of space at the Salt Lake Public Health Center located at 610 South 200 East, Salt Lake City, Utah. The lease is for a 10-year initial term with an option to extend for an additional 10 years, contingent upon mutual written agreement. The selected proposer must operate a healthcare facility offering direct physical or behavioral health services within the facility, coordinating with existing clinics such as WIC, STD, and Infectious Disease, using the provided space which includes seven exam rooms, consult rooms, a lab area, administrative offices, storage, a public restroom, and shared common areas. Services must be delivered during standard business hours, with extended hours available on Wednesdays until 6:30 PM. The tenant is responsible for all clinical operations, patient scheduling, billing, compliance with facility policies, and adherence to healthcare regulations including HIPAA and OSHA, as well as state law and GRAMA requirements for confidential records. Proposals are evaluated using a trade-off approach, with 70% of the score based on programmatic qualifications—including business type, professional licenses, service to underserved populations, and patient referral processes—and 30% on pricing. Scoring uses adjectival ratings from Excellent (5 points) to Unacceptable (0 points), with award not necessarily going to the lowest bid. Insurance requirements are stringent: $1 million per claim and $2 million aggregate professional liability, workers’ compensation as mandated by Utah law, and commercial auto liability, with the County named as an additional insured and subrogation waived. Lease payments are fixed at a minimum annual rate of $87,234, subject to a 3% annual escalation cap.
