The London Borough of Hounslow is a diverse and growing community in west London, with residents speaking over 188 different languages. Our population is increasing and living longer and, alongside rising levels of long-term conditions and dementia, demand for adult social care is both growing and becoming more complex. This contract is for the provision of home care services and carers' short breaks for residents aged 18 and over who have non-complex needs and are ordinarily resident within the London Borough of Hounslow (LBH). The service is jointly commissioned by the Authority and the West and North London ICB. The core purpose of the service is to enable people to live healthier, more independent lives by giving them greater choice and control over their care and support, maximising their social support networks, and strengthening community-based support. The service will also support unpaid carers by providing opportunities for planned short breaks, helping to sustain their caring role, maintain their wellbeing, and improve outcomes for both carers and the people they support.
