The solution will provide a single operational platform through which WM5G Ltd staff can manage referrals, patient workflows, consent, tasks, communications and service performance, replacing fragmented processes that currently rely on multiple supplier systems and manual administration. The procurement covers the software platform, implementation, integration, hosting, training, support and maintenance required throughout the contract term. The solution will act as the operational orchestration layer for the wider DDS service, providing a unified view of patient journeys while integrating with existing Home Readiness, Remote Monitoring and Asset Management services. It is intended to improve the efficiency, scalability and governance of discharge services while maintaining clear accountability and visibility across multiple organisations and care settings. The platform must support the complete discharge workflow, from patient identification and referral through triage, enrolment, discharge preparation, discharge, monitoring and completion.
