VA-248-08-RP-0420 is being issued as a request for proposals (RP). The North American Industry Classification System (NAICS) number is 334510 and the business size standard is 500. REQUESTERS MUST STATE THEIR BUSINESS SIZE ON EACH REQUEST. If a proposal for an equal product, specification sheets, manufacturer and part number of the item must be included in proposal. All Offerors must be registered at www.ccr.gov . See below for list of equipment required, OR equivalent. Far provision at FAR 52.211-6, Brand Name or Equal, applies to this requirement. The NF/SG VHS, Malcom Randall VA Medical Center, 1601 SW Archer Road, Gainesville, FL 32608 intends to negotiate a sole source purchase order with Bard Electrophysiology; the only known source capable of providing the Lab System Pro EP Recording System with the advanced AF mapping and analysis package with Electroview which creates an intuitive 3D image of complex arrhythmias. This system has enhanced visualization and the EPLink H.I.S. Interface option which utilizes the HL7 protocol. This cardiology diagnostic tool is used to determine where arrhythmias are originating. This is the primary diagnostic tool used in the Electrophysiology Lab. General description/performance requirements of the system are: Microsoft Windows operation system; expandable 80 channel amplifier with high signal quality; ability to record and display 80 channels simultaneously; (2) 40 channel junction boxes; template matching, T-wave subtraction, dominant frequency and 3D Electro view analysis with enhanced diagnostics; Afib cycle length analysis; laser jet printer; two (2) 21" LCD flat panel monitors for desk; two (2) 24" color monitors with VGA video splitters for remote monitoring; DVD patient data storage media; customizable report generator; enhanced Fluoro Capture; H.I.S. Hospital Interface with bidirectional communication; Review Workstation Software which includes 12 month warranty and software support; Installation and Training.This procurement is being conducted in accordance FAR 6.302-1, only one responsible source and no other supplies or services will satisfy the agency requirements. The resulting contract will be firm fixed price. This Notice of Intent is not a request for competitive proposals. No solicitation documents are available and telephone requests will not be honored. However, any firm that believes it can meet the aforementioned requirements is requested to submit clear and convincing evidence of their ability to comply with the above requirements to include FDA approval/compliance certification.
