99--ENDOSCOPY CONSIGNMENT AGREEMENT

The VA VISN 22 Contracting Activity, Long Beach California intends to enter into a sole source Consignment Agreement with Boston Scientific Corporation, Endoscopy Division, 100 Boston Scientific Way, Marlborough, MA 01752. This announcement constitutes the only notice issued; a separate written solicitation will NOT be issued. This Consignment Agreement is being conducted in accordance with FAR 6.302-1 only one responsible source and no other supplies or services will satisfy agency requirements, incorporates provisions and clauses as those in effect through Federal Acquisition Circular 2005-16. A justification for other than full and open competition has been prepared in support of this acquisition. Request for Quotation is VA262-12Q-0449. This Consignment Agreement is for as-needed procurement of multiple types/sizes of endoscopic stents maintained on-site VA Greater Los Angeles Healthcare System (GLA), West Los Angeles Campus (WLA) in advance of actual need for use and invoiced only when pulled for actual use in a medical procedure.
