FY 2026-27: Please note attachment with RFP details. We seek Management/Maintenance support, of new and existing network hardware. This may include Licenses, Software upgrades, bug fixes and security patches per hardware requirements. Bids including MIBS with localized onsite availability, when needed. A locally- available vendor will be prioritized. This submission includes request for on-site installation, please include that portion with your MIBS proposal. may be accepted. Unless specified in email by the school Tech Contact- communication, questions and bids concerning this 470 should be limited to EMAIL ONLY, directed to the person listed as TECH CONTACT listed in this 470 form. Entity reserves the right to choose to purchase any listed 2026 form 470 Installation, Hardware, Maintenance and/or Management services bundled or unbundled, in addition to initial configuration and technical support. All Hardware requests include associated Misc. eligible C2 items, including software and warranty. Bulk Upload Template: Final bids should include FULL SKU/Model number USAC bulk upload template (formerly known as the Item 21 attachment) for the Network Infrastructure project. Failure to include a template may result in a lower score. Provide FULL SKU/Model numbers in ALL final proposals. The following items are required to be considered a valid bid: (1) Initial contact MUST be made VIA EMAIL ONLY, to the TECHNICAL contact(s) listed. (2) The email subject line must reference the SCHOOL NAME and 470 NUMBER. (3) All responses MUST reference the services that you are bidding on. (4) All bids must contain your SPIN, Company Name, and Contact Information. (5) All bids must comply with all applicable state/local procurement laws, rules, and contract regulations. All accepted Bids \Contracts are fully contingent upon both eRate funding approval and support unless otherwise noted. In responding, Vendor agrees that complete or partial implementation of any executed proposals / contract related to this form 470 filing is fully at the discretion of the school/district, and may be rescinded on a per-item basis. Unless otherwise noted, initial proposal submission deadline is considered the 28-day bidding deadline in this 470- and initial bidder contact after the 28-day window will not be accepted until otherwise noted. All bids for Hardware/Software/Services described as FIREWALL, CACHING, UPS, or other 'partially eligible C2 items will require a full vendor breakdown of eRate C2 eligibility: if no eligibility is provided, vendor agrees by providing a bid to assume responsibility for unidentified ineligible costs. In the event budgeting is critical, Hardware proposals may be for both new and/or off-lease, refurbished, or otherwise preowned equipment. Any bid that fails to conform to the essential requirements of the invitation to bid, or contains basic or irrelevant answers may be rejected. Likewise, Failure to follow instructions, including multiple redundant auto-generated blanket bids sent from a single vendor, or generic/partial bids that are not specific to this application may be disqualified, and/or only the final submitted bid be considered. For security purposes, emailed links to an external site outside of the contents of an email may not be considered a valid bid, unless otherwise noted. NO LATE BIDS will be considered unless modifications to the Allowable Contract Date deadline are subsequently executed. Revised bids may be submitted during the bid window and subsequent vetting period when necessary, the last version will be considered the final document.

Proposals may be presented with a single year option up to a 5 year multi-year option, see narrative. Contracts are contingent upon eRate approval and support unless otherwise noted. Unless otherwise noted, initial proposal submission deadline is considered PRIOR to the Allowable Contract Date noted in this Form 470- Initial bidder contact ON or AFTER the 28-day window for Allowable Contract Date may not be accepted.
