This contract involves providing annual preventive maintenance services for optometry and ophthalmic equipment at the Crow/Northern Cheyenne Service Unit in Montana, with a performance period of 12 months starting October 1, 2025, and four subsequent one-year options. The work includes on-site inspections, cleaning, calibration, minor repairs, and replacement of small parts, all performed in accordance with OEM specifications, ANSI/AAMI EQ89:2015 standards, NFPA 99, and Joint Commission standards. The contractor must possess active OEM certifications, be an authorized distributor/service provider for relevant manufacturers such as Carl Zeiss Meditec, and provide qualified personnel with appropriate certifications. All services are to be scheduled in coordination with the clinics, performed during regular business hours, and documented through inspection stickers and detailed reports. Equipment not passing inspection must be tagged as “Do Not Use,” with failures documented and reported for corrective action. The contract emphasizes strict compliance with safety, quality, and regulatory standards, with final acceptance contingent upon the government’s review of completed work and documentation. The solicitation is a total small business set-aside under NAICS 811210, with award based on a best value trade-off considering price, technical capability, and past performance. Offerors are required to submit a comprehensive quote via email by September 9, 2025, including a completed price schedule as a separate attachment, a technical narrative limited to five pages, and evidence of OEM certification and authorized distributor status. The evaluation will weigh technical approach and past performance roughly equally with price, with the government reserving the right to award to other than the lowest priced offer if a superior technical proposal justifies a higher cost. Contract clauses incorporated by reference include FAR and HHSAR provisions covering security, confidentiality, labor standards, electronic invoicing via the Invoice Processing Platform, and other federal requirements. The contract performance will occur at designated clinics within the Crow Service Unit, with services scheduled to minimize disruption and ensure continuous equipment safety and functionality.
