This is a combined synopsis/solicitation for commercial items prepared in accordance with FAR Part 12. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued.


Refer to the attachments for additional details.


This requirement is for one (1) Firm-Fixed-Price (FFP) purchase order for supplies contract for dental equipment, located at Warm Springs, Oregon.  This will be a brand name or equal request for 3 each of the ConMed Micropower High Speed electric drill and 3 each of the ConMed Medium Bur Guard. The required delivery date is 10/31/2026. The delivery location and place of acceptance is 1270 Kotnum Road Warm Springs, Oregon 97761 F.O.B. Destination. (See attachments for details).

QUOTATION INSTRUCTIONS


Award will be made to the responsible Quoter determined to be eligible for award in accordance with FAR Subpart 9.1, Responsible Prospective Contractors. Award will be based on price and past performance.


Quoters shall submit all information and documentation required by this solicitation and its attachments. Refer to the attachments for additional details regarding the requirements of this solicitation.


As part of its quotation, each Quoter shall complete, sign, and submit the applicable Indian Economic Enterprise (IEE) form provided with this solicitation.


Failure to submit the required IEE form with the quotation may result in the quotation being excluded from consideration.


Quotes are due no later than August 28, 2026, at 5:00 p.m. Pacific Time (PT).

BASIS FOR AWARD


Award will be made to the responsible Quoter determined to be eligible for award in accordance with FAR Subpart 9.1, Responsible Prospective Contractors. Award will be based on price and past performance.


The Government reserves the right to:


  • Make no award;
  • Evaluate quotations and make award without discussions;
  • Communicate with Quoters for clarification purposes, as necessary; and
  • Exclude from consideration any quotation that fails to comply with the solicitation instructions or submission requirements.

EVALUATION FACTORS AND REQUIRED QUOTE INFORMATION


Quoters shall submit sufficient information for the Government to evaluate each of the following factors. Failure to provide the requested information may result in the quotation being excluded from consideration.


Factor 1 – Relevant Past Performance


Quoters may submit up to three (3) recent and relevant past performance references for projects similar in scope, size, and complexity to the requirements of this solicitation.


Each past performance reference shall include:


  • Customer/organization name;
  • Customer point of contact, including telephone number and email address;
  • Contract or purchase order number;
  • Contract or project dollar value;
  • Description of the work performed; and
  • Period of performance.

The Government may use the information provided, as well as information obtained from other available sources, to evaluate the Quoter's past performance.


Factor 2 – Price


The Government will evaluate the total evaluated price based on the CLIN structure provided in Attachment No. 1 – Pricing Schedule.


The quoted price shall include all costs necessary to successfully fulfill the requirements of the solicitation, including the cost of the items, shipping, freight, handling, transportation, delivery, and any other incidental costs necessary to complete delivery to the designated delivery location(s). No additional charges will be permitted unless specifically authorized by the Government.


Quoters shall complete and submit Attachment No. 1 – Pricing Schedule with their quotation.


REQUIRED QUOTATION SUBMISSIONS


At a minimum, Quoters shall submit the following with their quotation:


  • Completed Attachment No. 1 – Pricing Schedule;
  • Relevant past performance information required under Factor 1;
  • Completed and signed Indian Economic Enterprise (IEE) form; and
  • Any other documentation required by the solicitation or its attachments.

Failure to submit required documentation may result in the quotation being excluded from consideration.
