This requirement is for one (1) Firm-Fixed-Price (FFP) purchase order for dental equipment, located at Warm Springs, Oregon. BASIS FOR AWARD

The Government reserves the right to:

Make no award;
Evaluate quotations and make award without discussions;
Communicate with Quoters for clarification purposes, as necessary; and
Exclude from consideration any quotation that fails to comply with the solicitation instructions or submission requirements.

EVALUATION FACTORS AND REQUIRED QUOTE INFORMATION

Quoters shall submit sufficient information for the Government to evaluate each of the following factors. Failure to provide the requested information may result in the quotation being excluded from consideration.

Factor 1 – Relevant Past Performance

Quoters may submit up to three (3) recent and relevant past performance references for projects similar in scope, size, and complexity to the requirements of this solicitation.

Each past performance reference shall include:

Customer/organization name;
Customer point of contact, including telephone number and email address;
Contract or purchase order number;
Contract or project dollar value;
Description of the work performed; and
Period of performance.

The Government may use the information provided, as well as information obtained from other available sources, to evaluate the Quoter's past performance.

Factor 2 – Price

The Government will evaluate the total evaluated price based on the CLIN structure provided in Attachment No. 1 – Pricing Schedule, which quoter must utilize for quote submission.

The quoted price shall include all costs necessary to successfully fulfill the requirements of the solicitation, including the cost of the items, shipping, freight, handling, transportation, delivery, and any other incidental costs necessary to complete delivery to the designated delivery location(s). No additional charges will be permitted unless specifically authorized by the Government.

Quoters shall complete and submit Attachment No. 1 – Pricing Schedule with their quotation.

REQUIRED QUOTATION SUBMISSIONS

At a minimum, Quoters shall submit the following with their quotation:

Completed Attachment No. 1 – Pricing Schedule;
Relevant past performance information required under Factor 1;
Any other documentation required by the solicitation or its attachments.

Failure to submit required documentation may result in the quotation being excluded from consideration.
