Introduction. This is a non-personal services contract to provide Elevator and Chairlift Preven-tive Maintenance (PM), repair, test, inspection and certification services. The Government will not ex-ercise any supervision or control over the contract service providers. The Contractor shall perform to the standards specified in this Performance Work Statement (PWS). Contract service providers shall be accountable solely to the Contractor who, in turn is responsible to the Government. The Contrac-tor shall provide all personnel, equipment, supplies, facilities, transportation, tools, materials, supervi-sion, and other items and non-personal services necessary as defined in this PWS. The Contractor is responsible for providing all parts required for Preventive Maintenance Orders (PMOs) and all repair parts required for Demand Maintenance Orders (DMOs).


Background. The United States Army Garrison Hawaii (USAG Hawaii) requires preventive maintenance, repairs, testing, inspection and certification services for elevators and chairlifts at Army installation on the Island of Oahu, Hawaii. Technical Exhibits (TE) 1 through 6 lists the current inven-tory of elevators and chairlifts for this requirement. The number of elevators and chairlifts may in-crease or decrease, the Government may negotiate changes and modify the contract accordingly.


Objectives. The Contractor shall ensure all work accomplished in the performance of this PWS meets all applicable Federal, State, and Local Laws, Regulations and Directives to include, but not limited to, the publications in Section 7, Related Documents. The Contractor shall provide all neces-sary labor, management, supervision, materials, tools, equipment, supplies and transportation neces-sary to perform the services as specified herein this Performance Work Statement (PWS).

ADDENDUM TO SECTION L (INSTRUCTIONS TO OFFERORS FAR 52.212-1):
Pursuant to FAR 12.205, the provision at FAR 52.212-1 is tailored and augmented as follows: Offerors shall prepare their quotations in accordance with (IAW) FAR 52.212-1 and this addendum. A complete quote must be received to be considered for award. Quotes that fail to address all requirements or follow all instructions may not be considered eligible for contract award.


A complete quote includes a response and submission to each of the following:


1. General Information: Offeror Business Name, Address, CAGE Code, and UEI number (ensure Representation and Certifications are up to date in SAM.gov). Include Business Size and Type of Small Business based on the applicable NAICS Code, Federal Tax ID, Primary Point of Contact (to include telephone and e-mail address), and payment terms (i.e. net30).


2. Technical Acceptability Submission: The Offeror shall submit, as part of its quote, a technical acceptability narrative not to exceed 10 pages with 12-point font, demonstrating the Offeror's thorough understanding of the scope of work and capability to perform the services outlined in the Performance Work Statement (PWS). The Technical Acceptability Submission should be a single, stand-alone document containing all information necessary for the Government to perform a complete technical evaluation. To facilitate evaluation, this document must be formatted to clearly address each of the technical acceptability subfactors outlined in
52.212-2 and the added addendum to 52.212-2. Offerors may submit pictures, brochures, or other pertinent technical information to demonstrate ability, provided the total narrative does not exceed the 10-page limitation. The submission must include the following items; (Sections 2.a and 2b will not count against the 10-page limit for the narrative portion):
a. Contractor & Key Personnel Certifications: Offerors must submit documentation demonstrating the company possesses all required certifications. Provide resumes and copies of current, valid certifications for the proposed Project Manager and all key technicians, demonstrating compliance with the requirements of PWS Paragraphs 2.5.4.1 and 8.2.1.1 (e.g., valid Elevator Mechanic License in the State of Hawaii, valid Qualified Elevator Inspector certificate issued by the National Association of Elevator Safety Authorities, OSHA).
b. Technical Approach & Management Plan: A narrative providing reasonable assurance that the firm can meet the stated requirements. The narrative must address: (1) A preventive maintenance plan and technical approach in maintaining the elevators and chairlifts sustaining their operational condition. The plan should include a preventive maintenance schedule listing the maintenance tasks to be accomplished and periodicity as required by the elevator manufacturer. The preventive maintenance plan should be inclusive of elevator and chairlift maintenance, repair, test, inspection and certification services; and (2) A staffing and management plan demonstrating the firm's capacity to handle a high volume of concurrent task orders across the Garrison.
c. Relevant Work Experience: Provide at least two (2) project examples from the last three (3) years that are similar in scope, magnitude, and complexity to this requirement (i.e., elevator preventive maintenance and repairs - not to include elevator modernization) using attachment 0001 - Work Experience Reference Sheet. For each project, provide a brief description of the scope, period of performance, total contract value, and a customer point of contact (name,phone, email).


3. Past Performance Submission: Quoters are not required to submit past performance information. In accordance with FAR Subpart 9.1 (Responsible Prospective Contractors), the Government will review past performance as part of its evaluation to determine contractor responsibility for award. The Government will consider past performance information, if available, from the web-enabled Supplier Performance Risk System (SPRS) to determine regarding the vendor's past performance history. The Government may also query other internal databases (e.g., CPARS, SAM.gov) at its discretion.


4. Price Quote: The Offeror shall submit complete and accurate pricing for each CLIN listed in the schedule to include the unit price, the extended price for each line item, and a total price in US Dollars ($). Offerors shall provide complete pricing for all CLINs (excluding CLINs 0008 - 0009 and associated SubCLINs).
a. All Options CLINs must be priced (excluding CLINs X008 - X009 and associated SubCLINs). CLINs 0008 - 0009 and associated SubCLINs, and all equivalent Option Year CLINs associated are Non-Standard Parts and are Not-Separately-Priced (NSP) at the base contract level. Pricing for these parts will be negotiated as Firm-Fixed-Price (FFP) at the individual Task Order level following a Fair and Reasonable Determination.
b. Offerors shall insert 'NSP' in the unit price and total amount fields for CLINs 0008 - 0009 and associated SubCLINs, and all equivalent Option Year CLINs. Do not propose any baseline prices, percentage markups, or hourly labor rates for these CLINs in your quote. All parts and labor for these requirements are NSP at contract award, and will be negotiated post-award on an individual FFP Task Order basis IAW PWS Section 8.7.1.
c. The Offerors shall build in a one (1) year warranty into their pricing structure IAW PWS Section 8.8.


Partial quotes will not be accepted.


5. Baseline Preventive Maintenance Schedule: The Offeror shall submit a Baseline Preventive Maintenance Schedule with their quote. Reference PWS Section 8.2.1 and Technical Exhibits 3 and 4 of this solicitation. Quotes that do not include a Baseline Preventive Maintenance Schedule may be rejected or considered ineligible for award.


6. SF 1449: The Offeror shall complete and sign the SF 1449 in its entirety, paying particular attention to Blocks 12, 17a, and 30a-c, which must be completed and signed by an authorized representative. All amendments must be acknowledged. Quotes that do not include a properly completed and signed SF 1449, including acknowledgment of all amendments, may be rejected or considered ineligible for award.

Request For Clarifications and Questions (RFC) Due Date: Must be received no later than July 24, 2026, at 08:00 AM Hawaii Standard Time (HST).

The Government reserves the right not to respond to questions concerning this solicitation received after the date and time specified for receipt of questions. A summary of questions received, and Government responses will be provided by amendment and posted to the SAM.gov.


Request for Quotation (RFQ) Due Date: Quotes must be submitted electronically through upload to the SAM.gov website. It is the responsibility of the Offeror to ensure quotations are received and acknowledged by the contracting officer or contracting specialist. Late submissions will be treated in accordance with the solicitation provision at FAR 52.212-1(c).


52.212-1(c) Late submissions, modifications, revisions, and withdrawals of offers.


(1) Offerors are responsible for submitting offers and any modifications or revisions to the Government office designated in the solicitation by the time specified in the solicitation.


(2) Any offer, modification, or revision received after the time specified for receipt of offers is "late" and will not be considered unless it is received before award is made and the Contracting Officer determines that accepting the late offer would not unduly delay the acquisition. However, a late modification of an otherwise successful offer that makes its terms more favorable to the Government will be considered at any time it is received and may be accepted.


(3) If an emergency or unanticipated event interrupts normal Government processes so that offers cannot be received at the Government office designated for receipt of offers by the exact time specified in the solicitation, and urgent Government requirements preclude amendment of the solicitation or other notice of an extension of the closing date, the time specified for receipt of offers will be deemed to be extended to the same time of day specified in the solicitation on the first work day on which normal Government processes resume.


(4) Offerors may withdraw their offers by written notice to the Government received at any time before award.

ADDENDUM TO SECTION M (EVALUATION - COMMERCIAL ITEMS FAR 52.212-2)
FACTORS FOR AWARD


The Government intends to award a single Firm-Fixed-Price Requirements contract resulting from this solicitation. The Government will evaluate offers and make an award decision considering technical acceptability, price, and other information available to the Government, including past performance information obtained from CPARS or other sources.


The Government will evaluate offers based on two factors:


Factor 1 - Technical Acceptability, and


Factor 2 - Price


Past Performance is not an evaluation factor but will be reviewed as part of the Contracting Officer's responsibility determination under FAR Part 9.


For an Offeror to be considered for award, the offer must receive an "Acceptable" rating for the non-price factor of Technical Acceptability. An offer receiving a rating of "Unacceptable" will not be eligible for award.


Factor 1 - Technical Acceptability
The Government will evaluate the Offeror's Technical Acceptability Submission on an "Acceptable" or "Unacceptable" basis. To be rated Acceptable, the quotation must clearly demonstrate all of the following:
a. Contractor & Key Personnel Certifications: Offerors must provide copies of active, verifiable professional certifications (e.g., NAEC Certified Elevator Technician [CET], NEIEP, or equivalent state/federal licenses) for all proposed key personnel and field technicians. This is to demonstrate the offeror's ability to comply with the certification and submission requirements outlined in PWS Paragraphs 2.5.4.1 and 8.2.1.1. A failure to provide verifiable certifications for all proposed personnel with their quote will be determined technically unacceptable and ineligible for award.
b. Technical Approach & Management Plan: Offerors must submit the following documents with their quote:
1. A Baseline Preventive Maintenance (PM) Schedule in accordance with PWS Paragraph 8.2.1.
2. A Baseline Annual Testing Schedule mapped to the historical anniversary dates provided in PWS Technical Exhibits 3 and 4.
3. A Staffing & Management Plan demonstrating the capacity to handle concurrent task orders.
c. Relevant Work Experience: Submission of at least two (2) relevant work experience examples from the last three (3) years that demonstrate successful performance of similar projects. Failure to clearly demonstrate compliance with any of these requirements will result in the quotation being determined technically unacceptable and ineligible for award.


Factor 2 - Price
The Government will evaluate the total overall price for completeness and to determine if it is fair and reasonable in accordance with FAR 12.204(a). The Government expects competitive quotes and will use those quotes to determine price reasonableness. If competitive quotes are not received, the Government reserves the right to utilize other methods available under the FAR.


Quoters shall provide complete pricing for all CLINs (except CLINs 0008 - 0009 and associated SubCLINs, and all equivalent option CLINs of X008 - X009 and associated SubCLINs), which are Not-Separately-Priced and are excluded from the total evaluated price. Quoters shall not provide pricing for these CLINs.


Completeness. Each Quoter is required to provide complete pricing for each contract line item (CLIN) identified. This section must be mathematically accurate and include all CLINs and the associated option periods that the Quoter identifies in their quote.


Unbalanced Pricing. The Quoter shall ensure the pricing of contract line items are not significantly over or understated, which may increase performance risk or result in unreasonably high prices.


Options. As part of the price evaluation, the Government will evaluate the Option to Extend Services under FAR clause 52.217-8 by adding one-half of the Quoter's final Option price to the Quoter's total price. The final award evaluation will include the Base, all four (4) Option years, and the potential six (6) month extension FAR 52.217-8, Option to Extend Services.


Total Evaluated Price. Except when it is determined in accordance with FAR 17.202 not to be in the Government's best interest, The Government will evaluate offers for award purposes by adding the price of the Base, all four (4) Option years as contained in FAR 52.217-9, Option to Extend the Term of the Contract, and the potential six (6) month extension under FAR 52.217-8, Option to Extend Services. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced in relation to level of effort required. Evaluation of options shall not obligate the Government to exercise the options(s).


Responsibility Determination (Not an Evaluation Factor)

Prior to award, the Government reserves the right to conduct a responsibility determination in accordance with FAR Subpart 9.1. As part of this determination, the Government may review past performance information for the apparent successful offeror from sources such as CPARS and the Supplier Performance Risk System (SPRS) to assess performance risk. An offeror without a record of relevant past performance will not be evaluated favorably or unfavorably.
