G004--Emergency Residential and Case Management Services- VA New Jersey Health Care System- Health Care for Homeless Veterans (HCHV) Program

The Department of Veterans Affairs is seeking a contractor to deliver emergency residential housing and comprehensive case management services to up to 30 homeless Veterans under the Community Based Health Care for Homeless Veterans (HCHV) program, specifically for the VA New Jersey Health Care System. Services must be provided at a facility compliant with ADA and NFPA Life Safety Code standards, ensuring a safe, sober, and private environment with 24/7 on-site security, at least one CPR-certified staff member, and a clinical supervisor available around the clock with mandatory monthly on-site visits. Case management must address permanent housing placement, financial planning, recovery support, employment assistance, medication management, and transportation to treatment goals, maintaining a case manager to Veteran ratio not exceeding 1:20. The contract is structured as an indefinite quantity, single-award, commercial services procurement with a base period from October 1, 2026, through September 30, 2027, and four one-year option periods extending the total contract duration to a maximum of five years. The Government may also extend the period of performance for up to six months under FAR 52.217-8 or extend the contract term under FAR 52.217-9. All services are reimbursed at an all-inclusive daily rate per Veteran bed day, with pricing based on an estimated 10,950 bed days per year across the base and all options. The contractor must comply with all clauses under FAR Part 12, including 52.212-4 (Commercial Terms), 52.216-22 (Indefinite Quantity), 52.252-2 (Clauses Incorporated by Reference), and 52.222-90 (Addressing DEI Discrimination), while also adhering to VA-specific requirements such as VAAR 852.237-70 on indemnification and medical liability insurance. Mandatory certifications include compliance with the Immigration and Nationality Act, exclusion from the OIG List of Excluded Individuals/Entities, and adherence to the Privacy Act of 1974 and FIPS 140-2 encryption standards for handling VA-sensitive data. Annual VA information security and privacy training must be completed by all staff, with certificates submitted within one week of contract initiation.
