STATEMENT OF WORK FIRE DAMPER INSPECTION AND TESTING 1. Scope. The intent of this one-time service is to inspect and test multiple building s Fire Dampers (247 dampers) for regulatory compliance on the Biloxi VA Campus as described below within the 90 days following the Notice to Proceed. 2. Specifications/Characteristics: Minimum technical specs must be provided to meet the requirements of the GCVHCS.
Campus Overview - The 247 Fire Dampers for inspection and testing are in the Patient Buildings of 3, 30, 14, 15, and 25 and the Non-Patient Buildings of 1, 2, 5, 19, and 28 on the Biloxi VAMC Campus. See Chart 1. These Fire Dampers assist fire prevention from the buildings and communicates with the current installed Fire Alarm System. See Table 1.
All painted or damaged fusible links must be replaced with a link of same size, temperature, and load rating.
The location of each fire damper and access panel to be verified on each drawing and then updated on each drawing in accordance with NFPA guidelines.
All inspected fire dampers must have a visible label that corresponds to an Item number on the list, inspection date, condition, and inspector.
Each building inspected will have its own inspection documentation with 2 pictures minimum per damper showing damper itself and general comments affecting damper operation.
Each damper on the final report at a minimum shall state Pass / Fail.
Contractor shall furnish within the 90 days, the red-lined AutoCAD 2000 / Adobe PDF drawing for each building on a CD and on 11 x 17 sheet. All pictures taken are to be labeled, organized by building and floor, and provided on a CD.
Contractor shall furnish a formal Fire Damper Report on CD by building of all dampers inspected with inspectors signature. 3.0 Applicable Codes, Standards and Regulations 3.1. All work shall be done in accordance with the current National Fire Protection Agency (NFPA) Code requirements. 3.2. The Joint Commission, the latest applicable National Fire Protection Association (NFPA)  standards, and Occupational Safety and Health Administration (OSHA) regulatory requirements.  Testing is specific to 4 year frequency test for fire pump/smoke pump in facility. 3.2.1. NFPA 80 Standard for Fire Doors and Other Opening Protectives 3.2.2. NFPA 90A Standard for the Installation of Air Conditioning and Ventilating Systems 3.2.3. NFPA 101 Life Safety Code 3.2.4. NFPA 105 Standard for Smoke Door Assemblies and Other Opening Protectives 3.2.5. NFPA 5000 Building Construction & Safety Code 4. Comprehensive Fire/Smoke Damper Inspection Tasks  Contractor shall: 4.1. Furnish all labor, tools, and equipment to inspect fire and smoke dampers at Biloxi VAMC Campus. 4.2. Use commercially reasonable efforts to locate and, once located, operate all dampers provided in list by Contracting Officer s Representative (COR) upon commencement of inspection.  4.3. Include documentation of all tested devices and must identify as a minimum which includes date inspected, unique device identification, device location, status (Pass or Fail), the type and quantity of devices tested in each building, and a consolidated deficiency finding list per building.  4.4. Coordinate with the COR, Air Conditioning Equipment Mechanic, Electronics Mechanic and notify local fire department that the fire alarm system is in test mode due to the inspection of the smoke and/or combination dampers. 4.5. Locate, access and survey area of dampers.  4.6. Clear each damper of any debris that would impede such dampers normal operation.  4.7. Check for penetrations. 4.8. Ensure no interference to operation due to: (rusted, bent, misaligned, or damaged frame or blades, or defective hinges or other moving parts). 4.9. Inspect all linkage and pivot points for freedom of movement and condition.  4.10. Manually activate actuators on electric smoke and/or combination dampers to verify proper operation. 4.11. Lubricate all moving parts on each damper. 4.12. Check operation of the latch if damper is provided with a latch. 4.13. Reset fusible links on fire dampers, to verify such fire damper fully closes and if necessary, replace any fusible link that is compromised (per NFPA code requirements). 4.14. Replace springs and/or access handles for the fire/smoke dampers as necessary. 4.15. Identify each unidentified unit location with a ceiling grid damper label. 4.16. Identify on drawings the location of each damper, along with placement of identification stickers on ceilings or other location to assist in future location of such dampers; 4.17. Authenticate compliance utilizing a customized Universal Product Code (UPC) label applied directly to all damper units, specifying reference number and inspection date. 4.18. Create a comprehensive inventory and repair list for all inspected damper locations. 4.19 Thoroughly clean all areas with a high-density high efficiency particulate air (HEPA) filtration system. 4.20. Complete all pertinent damper unit repairs as deemed necessary (linkage adjustments, damper unit mounts, flex installations). 4.21. Contractor shall be able to perform such repair or replacement if dampers are in need of either replacement or repair, the scope of repair or replacement shall be determined by the project manager and/or contractor technician. A list of defective dampers shall be generated by the contractor and provided to the COR and Contracting Officer for approval. 4.22. Provide a report on conclusion of performance of evidence of inspection, repair, or replacement to include a listing of the location of each damper, damper s VA established ID number, and whether such damper passed or failed; if a damper failed, a detailed explanation of such failure.    4.23. Repair exceptions pertain to access door installations and new damper unit installations as determined by Fire and Smoke Damper Inspection. 4.24. Summary of 4 year fire/smoke damper testing: a) Physically access each damper b) Remove fusible link in damper c) Cycle the damper to verify full closure d) Test smoke damper actuators e) Replace fusible link with new one f) Document pass/fail results 4.25. Purpose of testing ensures that each damper will close to stop fire/smoke from traveling through ducts or walls. 5. Repair Work  5.1. The contractor shall inspect and provide documentation of fire damper functionality. The contractor shall make any adjustments necessary to the fire damper linkages and brackets to correct operation of the damper. Replace any links found to be damaged, painted or otherwise compromised or unacceptable with a link of the same size, load and temperature rating. 5.2. Repairs beyond the scope of fusible links: If additional repairs are required to make a damper operational, provide the nature of the repair and a cost estimate in the inspection summary report. 5.3. Any work that will result in additional charges MUST be approved by the Contracting Officer, in writing, BEFORE such work begins.  Any additional work performed by the Contractor without the prior, written approval of the Contracting Officer shall be at no additional 6.
