INTENT TO SOLE SOURCE

THIS IS A NOTICE OF INTENT TO AWARD A SOLE SOURCE CONTRACT AND IS NOT A REQUEST FOR COMPETITIVE QUOTES. Reference #: N0025917N0100. Naval Medical Center San Diego intends to award a firm fixed price sole source contract under the authority of FAR 13.106-1(b)(1)(i), only one responsible source, to: Alcon Laboratories, Inc.; 6201 South Freeway, Fort Worth, Texas 76134-2099. The objective of this sole source award is to Infinity Phaco Custom Packs and other lenses. Infinity Phaco Custom Packs must include: 1. Custom case of 10 packs of active FMS w/ 0.9 mm infusion sleeves 2. (10) 0.3 mm Polymer I/A Tip 3. (10) Slit knife 4. (10) Alcon Custom Packs Salient Characteristic of required Alcon Lenses and supplies: 1. Astigmatism Intraocular Lens 2. High cylinder power Intraocular Lenses 3. Sterile Ultraviolet and Blue Light Filtering 4. Acrylic Foldable Toric Aspheric Optic 5. Single- Piece Posterior Chamber Lenses 6. Biconvex toric aspheric optic 7. Multiple sizes must be available 8. Micro-embossed plastic 9. Single fluid catch bag 10. Multiple sizes of eye pak drapes 11. No pouches Alcon Laboratories is the only manufacturer that can provide these salient characteristics mentioned with the products above. The Infinity Phaco Custom Packs is the suitable option for this patient requiring optical surgery. Alcon Laboratories, Inc. is the sole manufacturer of the needed items. The NAICS code for this requirement is 339115; the small business size standard is 1000 employees. This notice of intent is not a request for competitive proposals and no solicitation document exists for the requirement. Sources interested in responding to this notice are required to submit a capability statement that includes management and technical data and cost information, in sufficient detail and with convincing evidence that clearly demonstrates the capability to perform the required work. Capability statements shall not exceed 5 (8.5 x 11 inch) pages using a font size no smaller than 10-point. All capability statements received by the due date of this notice will be considered by the Government. A request for documentation or additional information or submissions that only ask questions will not be considered as an affirmative response. A determination by the Government not to compete based on responses to this notice is solely within the discretion of the Government. Information received will be considered solely for the purpose of determining whether to conduct a competitive procurement or to proceed with a sole source purchase order.
