This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii) The Navajo Area Indian Health Service (IHS) intends to award a fixed-price commercial item contract in response to Request for Proposal (RFP) KSU-KG17-0007. This solicitation is issued on a SMALL BUSINESS SET-ASIDE. (iii) The solicitation documents and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-16. (iv) The associated North American Industry Classification System code is 339115 and the small business size standard is 1,000. (v) PRICE/COST SCHEDULE - SEE FEE SCHEDULE. (vi) PURPOSE OF THE CONTRACT: Task Description: Contractor shall provide Optical Coherence Tomographer. See Attachment for Specifications. (vii) Date and Place of Delivery: Kayenta Health Center, U.S. Highway 160, South of Mile Post 394.3, Kayenta, AZ, 86033. (viii) Specifications: See RFP. (ix) Evaluation Factors: The Government reserves the right to issue a single award or multiple awards to the schedule contractor(s) whose quote represents the best value as defined by FAR 2.101.
