The Nashville Area Office of the Indian Health Service is requesting proposals for Licensed Clinical Social Worker services at the Micmac Service Unit clinic 4-8 days per month. Documents are attached that constitute a request for proposal (RFP) numbered: 285-15-RFP-0022. The standard form (SF-1449) must be completed along with any other signature blocks for consideration. A detailed statement of work (SOW) starts on page three of the RFP document. Please read the SOW section in detail and base your proposal upon the information given therein. Please fill out all of the required information and submit your proposal back to our office by August 7, 2015.
