SYNOPSIS Nurse Case Management FY19-FY20 with 2-year Options to Extend Solicitation number: W912JB-19-Q-0016 SYNOPSIS Title: Nurse Case Management Services Restriction: Competitive 8(a) SBA program NAICS: 541611 Administrative Management and General Management Consulting Services Size Standard: $15 million Location: JFHQ National Guard, Lansing, Michigan 48906 Issue date: tentatively 10 June 2019 Closing date: tentatively 10 July 2019 Award date: tentatively 10 August 2019 General Description: This pre-solicitation synopsis is to provide notice to industry of the Michigan Army National Guard's intent to issue a RFQ (Request for Quotation) for commercial services utilizing Simplified Acquisition Procedures in accordance with FAR Parts 12/13. This requirement is a 100% competitive set-aside for the 8(a) program. Offers from other than 8(a) program participants will not be considered. This requirement will be a competitive request for quote (RFQ). The RFQ is anticipated to be released on or about 10 June 2019 using solicitation number W912JB-19-Q-0016 and will be posted to FBO. Offerors will be given 30 days to submit a quote from the date of the issuance of the solicitation. The government intends to release a solicitation for qualified 8(a) companies to maintain health readiness records in compliance with Health Readiness Record (HRR). In addition, case management support is needed to manage open cases and referrals. The Performance Work Statement for the technical details will be provided with the solicitation posting. Records tracking will be for the counties of Crawford, Ingham, Jackson, Kent and Wayne for approximately a total of 8,100 soldiers. The resultant award will be a single award firm fixed priced contract for a base period of performance for (8) eight months with two 1-year (12 months) option periods in accordance with 52.217-9. The government will consider an additional extension of up to 6 months in accordance with 52.217-8. Quotes will be evaluated inclusive of options. The award will be based on price, technical approach, past performance. All evaluation factors are approximately equal and an award will be made that will result in the best value to the government. All information required for submission of an offer will be identified in the solicitation. Solicitation W912JB-19-Q-0016 will be posted electronically on or about June 10, 2019, at https://www.fbo.gov. All responsible sources that meet the terms and conditions of the solicitation may submit a quote which shall be considered by the agency. The Government may not consider quotes that fails to meet all of the requirements, terms and conditions of the solicitation. Any costs incurred as a result of a response to this announcement shall be borne by the contractor and will not be charged to the government for reimbursement. Paper copies of the solicitation will not be available. You must register on FBO (https://www.fbo.gov) in order to receive notification of changes to the synopsis and solicitation. Closing of the solicitation is anticipated to be on or about July 10, 2019.
