Emailed quotes are the preferred method.STATEMENT OF WORK (SOW) Fuel Contract This statement of work (SOW) contains a recommended format for contracting a fuel contractor for services associated with IRT/SMASE 100 Mustang Road Window Rock, Az. 86515. It may be used by all persons and organizations that prepare, use, award, and administer these contracts. The standards describe the minimum acceptable standards for the resident mechanic and services rendered. When local conditions dictate, modifications should seek the highest standards attainable at a reasonable price. 1 GENERAL 1.1 SCOPE OF WORK: The organization shall provide the equipment necessary to deliver off-road diesel to the heavy equipment and/or fuel storage in 250 Gallon increments on site(s)/location at 100 Mustang Road Window Rock, Az. 86515. **This requirement is contingent on the availability of funds. 1.2 QUALITY CONTROL: The organization shall establish and maintain a complete quality control program for the performance requirements of this contract. SECTION 2 DEFINITIONS 2.1 Equipment: Fueling equipment/trucks that are capable of delivering fuel to the project work sites located 100 Mustang Road Window Rock, Az. 865152.2 Fuel: Off-Road Diesel SECTION 3 CONTRACTOR FURNISHED ITEMS AND SERVICES 3.1 GENERAL: The contractor shall provide all the equipment and labor required to perform this statement of work. 3.2 MINIMUM QUALITY STANDARDS: All fueling operations shall be delivered with the equipment designed for that purpose. 3.3 VEHICLES: The contractor shall provide any and all vehicles necessary to fulfill the requirements of this contract. 3.4 All vehicles shall be operated in accordance with project site traffic regulations. 3.5 EQUIPMENT: The contractor shall provide all associated mechanical equipment necessary to fulfill the requirements of this contract. Section 4 SPECIFIC TASKS 4.1 The following identifies, but is not limited to specific tasks for the Fuel contract: 4.2 The fueling contractor will be responsible for providing the designated fuel to 100 Mustang Road Window Rock, Az. 86515 4.3 Be able to deliver fuel within 18 hours of request. 4.4 Provide the necessary equipment to off load the fuel either into designated fuel tanks or into the designated heavy equipment. 4.4 Provide services starting at time of request upon awarded the fuel contract through 30 September 2012. 4.5 Hours of operation for delivery will be within contractors designated hours of delivery. However, pending mission requirements IRT SMASE may request services outside of contractors designated business hours. 4.6 Be prepared to supply up to 8000 gallons of off road diesel fuel between 1May12 and 30SEP12. First delivery will be approximately 250 gallons and will be required NLT 1 May 2012. Be available to support additional fueling requirements if required/requested. Delivery address: 100 Mustang Road Window Rock, Az. 86515. SECTION 5 HAZARDOUS REQUIREMENTS 5.1 The contractor shall be responsible for delivering fuel within the requirements of local/state/federal laws governing. 5.2 The contractor shall be responsible for any accidents caused by the contractor while on site delivering fuel in support of IRT/SMASE. 5.3 The contractor shall be responsible for the clean-up/removal of any spillage of hazardous material while on site delivering fuel at The Summit in support of IRT/SMASE. (X) FAR 52.212-3 (Alt 1), Offerors Representations and Certifications, Commercial Items. Offerors shall include a completed copy as part of their quote. A copy of the Offerors Representations and Certifications may be obtained from (http://farsite.hill.af.mil). However, vendor must complete the Reps and Certs as stated in section (IX) above at http://orca.bpn.gov. (XI) FAR 52.212-4, Contract Terms and Conditions is incorporated by reference and applies to this acquisition with the following addendum: DFARS 252.204-7004 Required Central Contract Registration, Commercial Items. (XII) FAR 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders (DEVIATION).
