Notice of Intent to Sole Source Award


THIS IS A NOTICE OF INTENT TO AWARD A SOLE SOURCE CONTRACT AND IS NOT A REQUEST FOR COMPETITIVE QUOTES.


The Gallup Service Unit (GSU), Indian Health Service, plans to award a firm-fixed-price sole source purchase order under the authority of Federal Acquisition Regulation (FAR) Part 6.302-1, Only One Responsible Source and No Other Supplies or Services Will Satisfy Agency Requirements to SW Logistics LLC dba UltraShred, 1823 Commercial St. NE, Suite B, Albuquerque, NM 87102-1598 (UEI: G269GSP6ZQN3).


The North American Industry Classification System (NAICS) code for this requirement is 561990 – All Other Support Services, which has a small business size standard of $16.5 million. The object of this sole source award is to obtain on-site, secure document destruction services that are compliant with the Health Insurance Portability and Accountability Act (HIPAA) and the Privacy Act of 1974. This must include proof of the mandatory NAID certification as well as a completed INDIAN HEALTH SERVICE BUY INDIAN ACT INDIAN ECONOMIC ENTERPRISE REPRESENTATION FORM verifying the firm's ISBEE status in accordance with the Buy Indian Act. If parties choose to respond, any cost associated with the preparation and submission of data or any other cost incurred in response to this announcement is the sole responsibility of the respondent and will not be reimbursed by the Government
