Amendment No. Two for solicitation 12-247-SOL-00069 regarding Pest Elimination Control Services extends the closing date to July 31, 2012, at 12:00 PM. It introduces strict limitations on subcontracting under FAR 52.219-14, requiring small business concerns to perform a minimum percentage of the work based on the contract type, such as 50 percent for services and 15 to 25 percent for construction. All subcontracting requires prior written approval from the Contracting Officer, and contractors are prohibited from hiring entities listed on the GSA's Lists of Parties Excluded from Procurement and Non-procurement Programs. The amendment mandates a site visit at the Phoenix Indian Medical Center in Arizona on July 24 to allow offerors to inspect facility conditions. Contractors are held fully responsible for the actions of their subcontractors and must provide a complete list of all scheduled subcontractors within ten days of the contract award. To be considered, offerors must sign and submit the required SF-30 forms and acknowledge receipt of the amendment by the specified deadline.
