This procurement is conducted under Simplified Acquisition procedures pursuant to the authority of Federal Acquisition Regulation (FAR) Subpart 13.5, Test Program for Certain Commercial Items (10 U.S.C. 2304(g) and 2305 and 41 U.S.C. 253(g) and 253a and 253b) and FAR Part 12, Acquisition of Commercial Items (Title VIII of the Federal Acquisition Streamlining Act of 1994 (Public Law 103-355)). The solicitation is restricted to 100% Small Business concerns. The Indian Health Service (IHS) intends to award a CHS requirements type commercial item contract in response to Request for Quotation (RFQ)-10-10-041-REL for Prescription Eyeglasses for the IHS facilities in the Billings Area. As set forth in FAR 52.216-18, Ordering (OCT 1995), the Contract Health Service (CHS) Offices at the Service Units will request the supplies and obligate the funds on the IHS-T-843-1A Purchase-Delivery Order for Health Services. Performance of the contract shall be for 12-months with four 12-month options.
