(1) Extend the offer Due Date and Local Time for Combined Synopsis/Solicitation HT940626QE060 to 10:00 a.m., Eastern Time, to 25 September 2026.


(2) Extend the deadline for submitting questions, as follows:


All questions must be submitted in writing via email NO LATER THAN 10:00 A.M., Eastern Time, 23 September 2026. Questions must be sent via EMAIL to Derrick Walker, Contract Specialist, at derrick.w.walker.civ@health.mil, and Jessica Horst, Contracting Officer, at Jessica.l.horst5.civ@health.mil. SUBJECT: Solicitation No.: HT940626QE060. NO telephonic questions will be accepted.


(3) Due to a system error which resulted in Amendment #0002 not being viewable by potential offerors in SAM.gov. Amendment #0003 publicizes responses to questions submitted prior to the original solicitation closing date of 17 August 2026. Responses to questions submitted are provided in Attachment 1: Combined Synopsis/Solicitation HT9406QE060.

The Defense Health Agency Contracting Activity (DHACA), Healthcare Contracting Division - Southeast (HCD-Southeast), on behalf of the 6th Medical Group (MDG), MacDill Air Force Base (AFB), Florida, has a requirement for preventive maintenance on the Government-owned designated medical equipment - Two (2) STERIS AMSCO 400S Small Steam Sterilizers and 2 STERIS AMSCO 2532 Single-Chamber Washer/Disinfectors – located at the 6th MDG, Dental Facility, MacDill AFB.  Reference the Performance Work Statement (PWS), section 7.1., for a full list of the Government-owned medical equipment.


The Contractor shall provide preventive maintenance, to include corrective maintenance (CM), calibration, diagnostics, installation of replacement parts, and other actions taken to return the equipment to original manufacturers specifications following a failure, damage, or other incident that causes the equipment to no longer perform within the original specifications. 


The contractor must be an Original Equipment Manufacturer’s (OEM) certified service provider.  Preventive maintenance and corrective maintenance, to include all periodic planned maintenance and unscheduled repair, on the medical equipment must be in accordance with OEM specifications and in compliance with ISO 9001:2015 standards.
