Page 2 of 2 The Department of Veterans Affairs, Veterans Health Administration, Network Contracting Office 17, intends to award a firm-fixed priced contract (Period of Performance: Base plus four option years) to Decipher Corp. for the procurement of Decipher Genomic Prostate Cancer Testing. The testing will include: all related CPT codes, all labor, supplies, equipment, insurance, and supervision to provide the items. The off-site testing will be administered by the VA North Texas Healthcare System, Pathology & Laboratory Medicine Service, 4500 S. Lancaster Rd., Dallas, TX 75216. Item Description 0001 Provide sendout reference laboratory testing not performed in-house at VANTHCS' Pathology & Laboratory Medicine Service Lab. Test will encompass all CPT codes. Contractor will provide all labor, supplies, equipment, insurance, and supervision to provide the items. Items are to be ordered on an as needed basis exact quantities of each item cannot be determined in advance as they are based upon physician's orders. This test provides prognostic information to help guide and personalize the treatment of prostate cancer. Test predicts 5yr Metastasis and 10yr cancer-related mortality in localized prostate cancer. Total annual estimated test volume is 15. The proposed sole source procurement will be made by a pricing agreement with Decipher Corp., based upon pricing from previous contracts. The requesting activity states the off-site testing agreement is for genomic prostate cancer testing; required for the testing of post-surgery oncology patients. The sole source justification request submitted by the requesting activity stated that a sole source is required because they require genomic prostate cancer testing for patients who have previously undergone surgery to determine the likelihood of the cancer reoccurrence. This notice is not to be considered a request for quotations or proposals. A contract will not be awarded based on an offer received in response to this notice; however, all responsible sources may submit a capability statement, proposal or quotation, which shall be considered by the agency. Proposals or offers received will be used to determine if a comparable source is available and more advantageous to the government. If no affirmative written response is received within five (5) days from the publication of this notice, a contract will be issued to Decipher Corp., without any further notice. The anticipated statutory authority permitting other than full and open competition for the requirement is 41 U.S.C. 3304(a)(1), as implemented by FAR 6.302-1, Only One Responsible Source and No Other Supplies or Services will Satisfy Agency Needs. All responsible sources that feel they should be considered qualified to provide the above-mentioned products may submit a response to this notice (by the response due date shown above) to Susan.Flores2@va.gov. The information provided shall include detailed (and relevant) documentation to allow the government to perform adequate market research and make a proper determination. A determination by the government not to compete this proposed requirement based upon responses to this notice is solely within the discretion of the Government. Information received will be considered solely for the purpose of determining whether to conduct a competitive procurement. THIS IS NOT A REQUEST FOR COMPETITIVE PROPOSALS.
