Request for Applications - Consultant and Management Entity Approval

The Oregon Department of Human Services, through its Office of Aging and People with Disabilities, is issuing solicitation S-10000-00017902 to establish a resource list of approved consultants and management entities. These approved providers will offer consultation support to Residential Care and Assisted Living Facilities and Endorsed Memory Care Communities to help them improve care services, address operational deficiencies, and obtain higher-level licensure endorsements. The agency will not pay the consultants directly; instead, the facilities themselves are responsible for hiring and contracting with the approved consultants. For facilities with license conditions, the agency requires a contract with an approved consultant for at least one month prior to licensure and for at least six months following the initial license issuance. To be eligible, applicants must hold or employ a licensed residential care facility administrator and demonstrate at least three years of relevant management experience in Oregon within the last five years, including specific memory care experience if applicable. Applicants must not be excluded by the U.S. Department of Health and Human Services Office of Inspector General or appear on the OFAC Specially Designated Nationals list. Evaluation is based on a scoring system covering the scope of services narrative, staff qualifications, resumes, and technical requirements, with a minimum score of 100 required for placement on the approved list. Required submissions include five specific attachments covering identification, key personnel, business information, responsibility inquiries, and disclosure exemptions.
