The purpose of this contract would be to provide a comprehensive Therapeutic Shoe Program
The Contractor shall provide patient evaluations, measurements, fittings, dispensing, adjustments, modifications, replacements, and follow-up services for therapeutic shoes, custom or prefabricated inserts, diabetic socks, and other medically necessary diabetic foot supplies.
All therapeutic footwear and supplies provided under this contract shall meet applicable Centers for Medicare & Medicaid Services (CMS) Therapeutic Shoes for Persons with Diabetes coverage requirements and accepted industry standards.
