Full Description

The Phoenix Area Indian Health Service (IHS) is soliciting proposals from interested Native American Traditional Spiritual Licensed Counselor(s) and Traditional/Cultural Art Specialist, to provide services under a non-personal services contract to the clients of the Desert Visions Youth Wellness Treatment Center in Sacaton, AZ, located on the Gila River Indian Community Reservation. Experience with at-risk adolescents, diverse tribal groups, and experience in facilitating counseling with individuals and groups is required. All potential offerors shall be classified as a business entity. In addition, all potential offerors shall be subject to pre-security clearances prior to commencing services at the wellness center; the security clearance encompasses fingerprinting and a screen check through the Office of Inspector General (OIG) website. The anticipated period of performance is for a base year of twelve (12) months, beginning September 1, 2012 through August 31, 2013, with option to extend services for four (4) additional one-year option periods, or a total of five years. Multiple awards may be made. The Contractor shall meet all criteria contained in the Request for Proposal (RFP). The unit price shall be an all inclusive rate to include travel, lodging, per diem, fringe benefits, federal, state and local taxes, including all other costs pertinent to the performance of the contract.
