2. Statement of Work
BACKGROUND. This requirement, driven by VA Central Office and The Joint Commission (TJC) on Patient Safety, mandates that dental and medical instruments are no longer permitted to soak in solution overnight. It is imperative that all instruments are returned to the Sterile Processing Service (SPS) for sterilization on the same day they are used. To comply with this mandate, it is necessary to arrange motorized vehicle runs between the VA Greater Los Angeles Healthcare System (VAGLAHS) West Los Angeles (WLA) Campus and selected remotely located clinical facilities. These transport services will be scheduled from Monday through Friday, excluding holidays.
SCOPE. Contract Staffing: The vendor must provide minimum of six (6) motor vehicle operators (truck driver, class c) with valid Class C Driver s license issued by one of the fifty (50) States of the United States of America and 6 utility vans, Monday through Friday except holidays.
Vehicles: The vendor is required to provide their own vehicles, which must comply with Department of Transportation (DOT) regulations, Department of Health standards, and Veterans Health Administration (VHA) Directives 1116 pertaining to the transportation of reusable medical devices (RMD). Each vehicle must be equipped with a separate partition that distinctly segregates CLEAN RMD from CONTAMINATED RMD to prevent the mixing of contaminated instruments with sterile instruments. All vehicles must be insured.
Specific Tasks Motor vehicle drivers are required to perform courier runs between the VA West Los Angeles (WLA) campus and to specified remote locations for the delivery and pickup of dental and medical instruments in spill-proof containers. Each run will commence at the WLA Sterile Processing Service (SPS) located in Building 500, where the vehicles are parked. Drivers must complete a daily vehicle checklist, which includes checking fluids, brake lights, and other essential components.
In the event of any issues involving the transportation of reusable medical devices (RMD), the contractor must promptly notify SPS management. Each driver is responsible for documenting the temperature and humidity levels at both the point of departure and the destination. Logs for this purpose will be provided by the SPS management team. Additionally, SPS management must be contacted immediately regarding any non-conformance related to the transportation of RMD.
Drivers will proceed to the ground floor of Building 500, specifically to the rear loading dock of the Sterile Processing Service (SPS), where instrument containers are to be picked up. Upon returning from the remote destinations, the instrument containers must be delivered back to the SPS location in Building 500. Each work shift will consist of eight (8) hours per tour of duty, including a 30-minute lunch break. The remote locations are as follows: Outpatient Centers Sepulveda (SACC)/Los Angeles (LAACC) approx. 54 miles round-trip* Bakersfield Community Based Outpatient Clinic (BAK) approx. 212 miles round-trip Santa Maria Community Based Outpatient Clinic (SM) approx. 298 miles round-trip (*Two runs per day to this location; requires two drivers for this trip to eliminate overtime.) Ventura Community Based Outpatient Clinic approx. 112 miles round-trip. Antelope Valley Community Based Outpatient Clinic approx. 66 miles round-trip
Cleaning: The vehicles must be cleaned by the driver daily. The driver is responsible for thoroughly cleaning the interior of the van, beginning with the section labeled "CLEAN" and concluding with the "DIRTY" section. While performing the cleaning duties, the driver must wear gloves and use the hospital-approved disinfectant, Sani-Cloth AF3 Wipes. Additionally, the driver is required to document the completion of the vehicle cleaning in the log provided by the SPS management team each day.
Upon the pick-up and drop-off of Reusable Medical Devices (RMD), the temperature and humidity levels must be recorded in the log provided by the SPS management team to ensure package integrity. The temperature must not exceed 72°F and the humidity must not exceed 60%. If either the temperature or humidity are out of range, the driver must inform the end user(s) (clinic staff). The end user will then perform a risk assessment to ensure that the packaging has not been compromised. If any packaging is found to be compromised, the end user will contact the Sterile Processing Service management team and specify whether all packages need to be returned for reprocessing.
The drivers will be responsible solely for delivering medical supplies (instruments) related to Reusable Medical Devices (RMD) to the various clinics indicated above. The drivers will deliver Clean RMD and pick up Dirty RMD directly to and from specific clinical areas within each facility, such as the Eye Clinic, Podiatry, Radiology, Dermatology, etc. The driver must ensure that the end user signs both the Signature and Release to SPS form and the Signature and Release from SPS form to document the transfer of RMD.
Facility Tasks SAACC and LAACC Run Drivers: There are two runs scheduled for this trip with identical duties; the primary difference lies in the shift timings. The first shift runs from 7:00 a.m. to 3:30 p.m., while the second shift runs from 12:00 noon to 8:30 p.m. The departure for the first run should occur within one hour of the start of the shift. The departure for the later run is timed to arrive at the Los Angeles Ambulatory Care Center (LAACC) at approximately 3:45 p.m. to pick up instrument containers after the impacted clinics have closed.
These runs require stops at both the Sepulveda Ambulatory Care Center (SAACC) and Los Angeles Ambulatory Care Center (LAACC). For SAACC, instruments need to be delivered to the clinic at the start of their operating hours, which are 8:00 a.m.
Bakersfield Community Based Outpatient Clinic (BAK) run driver: The shift from 11:00 a.m. to 7:30 p.m. is designed to accommodate the later run, which is timed to reach Bakersfield (BAK) at approximately 3:45 p.m. for instrument container pickup after the impacted clinics have closed.
Santa Maria Community Based Outpatient Clinic (SM) run driver: The shift from 11:00 a.m. to 7:30 p.m. is structured to accommodate the later run, which is scheduled to reach Santa Monica (SM) at approximately 3:45 p.m. for instrument container pickup following the closure of impacted clinics.
Ventura Community Based Outpatient Clinic Ventura run driver: The shift from 2:00 p.m. to 7:00 p.m. is structured to accommodate the later run, which is timed to arrive in Ventura at approximately 3:45 p.m. for the pickup and drop-off of instrument containers following the closure of impacted clinics.
Antelope Valley Community Based Outpatient Clinic Antelope Valley run driver. The shift from 11:00 a.m. to 7:30 p.m. is structured to accommodate the later run, timed to reach Antelope Valley at approximately 3:45 p.m. for instrument container pickup after the closure of impacted clinics.
Other Mandatory Actions The contractor must provide a backup driver if a driver is not available for a specific route.
If instruments are lost in transit the contractor must replace the cost of the missing instrument(s).
In the event of an accident involving the vehicle(s), the contractor must immediately notify SPS leadership at West LA VA. Any replacement vehicle(s) must be evaluated by SPS leadership to ensure compliance with VHA/NPOSP directives.
All contract staff are required to be appropriately attired in the company uniform.
The contractor must provide their own equipment for transporting RMD from the facilities to their vehicles (i.e. dolly).
Contractor must cover all vehicle maintenance and fuel costs.

The Government may determine that an [offer] is unacceptable if the option prices are significantly unbalanced or if the [offered] price for the basic requirement or any option exceeds the amount of funding available for any of those requirements.  Evaluation of options should not oblige the Government to exercise the option(s).
(d) A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer s specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before the award.    (End of provision)
