This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6 "Solicitation for Commercial Items," as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. Quotations are being requested and a written solicitation will not be issued. This solicitation number is RFQ-DC-14-00136 and the solicitation is being issued as a 100% SMALL BUSINESS SET-ASIDE competition Request for Quotation (RFQ) using FAR Part 13 Simplified Acquisition Procedures. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 05-68. The associated North American Industry Classification System (NAICS) Codes are 423450 "Medical, Dental, and Hospital Equipment and Supplies Merchant Wholesalers and 611699 "All Other Miscellaneous Schools and Instruction." The Environmental Protection Agency has approximately 4,135 employees who are located at the Federal Triangle Complex and several nearby buildings. The EPA provides training to approximately 200 to 300 of these employees in the use of life-saving equipment and practices annually. To further these efforts, the Safety Health and Environmental Management Division (SHEMD) is seeking a vendor to provide training and equipment for its headquarters buildings. The vendor shall administer an AED program that includes the lease of 102 automatic Cardiac Science Powerheart G3 AEDs (and their registration in Washington, D.C.), two AED training units, ten manikins, and medical direction. The training will include using the Automated External Defibrillator (AED), performing Cardiopulmonary Resuscitation, First Aid and Emergency Oxygen Administration, and familiarization with Bloodborne Pathogens. Further details regarding the requirement are set forth in the attached Statement of Work. A firm fixed-price contract is anticipated to result from award of this solicitation. The period of performance of this contract will include a base year and four (4) option years. In accordance with FAR 52.204-7(b)(1)Central Contractor Registration, all prospective awardees shall be registered in the CCR/SAM database prior to award, during performance, and through final payment of any contract or purchasing agreement resulting from this solicitation (www.sam.gov). All offerors are to include with their offers an affirmation that Offeror Representations and Certifications -Commercial Items, are complete for the NAICS code applicable in the SAM database.
