The NY Planning Council approved a new Care Coordination Services (CCR) service directive which was developed to address the many different needs of people with HIV (PWH) by helping them access medical care, treatment, and social services in New York City (NYC). The Directive for Aging PWH, approved by the Planning Council (PC) on October 28, 2021, was developed to improve health outcomes by treating the complex needs of aging PWH though health education; geriatric, psychiatric, and cardiology consultation; and referrals to specialty care. Services are tailored to help improve self-advocacy and management and to provide referrals for health care and supportive services. The Emergency Financial Services (EFS) service directive approved by the PC on December 5, 2024, was developed to address short-term, emergent financial needs for essential goods and services including utilities, housing, food (including groceries and food vouchers), and/or medicines for PWH in the New York eligible metropolitan areas (EMA).
