Employee Assistance Program

The Phoenix Union High School District is soliciting a comprehensive Employee Assistance Program (EAP) to serve approximately 3,173 eligible employees and their household members for the plan year running from July 1, 2027, through June 30, 2028. The contract may be extended for up to four additional one-year terms, totaling a potential five-year period. Required services include 24/7/365 toll-free crisis counseling, a minimum of six one-hour face-to-face visits per issue per year, unlimited critical incident stress management, and a minimum of 10 hours of orientation and educational seminars. Additionally, the contractor must provide a dedicated account management team, mediation services for HR referrals, quarterly utilization reports, and a program website for behavioral health and wellness information. Proposals are evaluated using a point formula system where cost accounts for 50% of the score, qualifications for 35%, and responsiveness for 15%. The District reserves the right to conduct interviews and negotiations to determine the most qualified firm. All submissions must be sent as a single PDF via the OpenGov portal by December 10, 2026. Contract administration is managed by the Chief Financial Officer, with payment terms set at Net 30 days. The contractor must adhere to strict federal and state regulations, including the Americans with Disabilities Act, the Davis-Bacon Act, and various environmental and labor standards, while maintaining a current W-9 form for payment eligibility.
