Enhancing Blueprint Transformation Network Capacity: Workforce Support and System Development through Learning Collabs

Vermont’s Department of Vermont Health Access is seeking proposals to enhance rural health care delivery through the Advanced Primary Care Learning Collaboratives initiative, funded by a $195 million Rural Health Transformation grant in Year One of a five-year program. Vendors are required to disclose any organizational or personal conflicts of interest, implement robust mitigation measures, and fully indemnify the State against breaches or unauthorized disclosures involving state-funded data. Workers must comply with Vermont Act No. 54 regarding proper employee classification and insurance, and subcontractors must be reported if the contract exceeds $250,000. All contractors must complete the Climate Change Considerations Certification for contracts over $25,000, reflecting the State’s commitment to sustainable procurement.
