Ground Ambulance Services at Ft. Polk, LA

The contractor shall provide Ground Ambulance Services at Fort Polk, LA in accordance with the Performance Work Statement included in the SF1449.
All offerors shall provide the following items: Please complete the SF1449: Blocks 17 (to include the CAGE code and the Unique Entity ID Number) and 30 A - C.
Please provide a statement that the offeror will hold its price firm for at least 90 days from the due date for receipt of proposals, subject to any changes that may occur as a result of discussions (if applicable).
Please complete the provisions, as listed in the solicitation. Alternately, complete all provisionsin System for Award Management (SAM) and provide confirmation of completion and accuracywith offer. Potential contractors must be registered in SAM to be eligible for award. The SAM website is http://www.sam.gov.
Proposals are due by no later than (NLT) 8:00 AM/CDT on 21 October 2026. Proposals shall be submitted via email to marc.a.flores3.civ@health.mil and salameya.paulouskaya2.civ@health.mil. Proposals received after the deadline will be considered untimely and will not be eligible for award.
Questions are due by NLT 8:00 AM/CDT on 24 September 2026. All questions must be submitted in writing to marc.a.flores3.civ@health.mil and salameya.paulouskaya2.civ@health.mil. Electronic submissions with files larger than 20MB will need to be submitted in separate emails. All emails, regardless of the size, need to be received by the Government server, by the stated date and time. It is the offeror's responsibility to ensure the Government receives the entire proposal

Factor 1. Technical: The technical proposal shall be addressed in sufficient written detail for the Government to determine if the Offeror understands the technical aspects of the Government's requirement. Offerors shall demonstrate how they will comply with the following subfactors:

Subfactor 1: Technical Capabilities:
- Offeror shall submit a detailed technical approach demonstrating its ability to perform all requirements of the Performance Work Statement (PWS), including the full range of ground ambulance services required under the contract.
- Offeror shall provide management and operational plans, including safety procedures, staffing, training, quality control, and transition/start-up timelines from contract award through full operational capability.
- Offeror shall provide a contingency plan addressing staffing shortages, vehicle breakdowns, severe weather, communications failures, emergency evacuations, and continuity of operations.
- Offeror shall demonstrate its capability to respond to multiple simultaneous emergency dispatches, peak workload periods, and installation training events while maintaining required response times.
- Offeror shall demonstrate its ability to provide and sustain the required staffing, vehicles, equipment, and operational resources necessary to support continuous 24-hour, 7-day, 365-day emergency medical services.
- Offeror shall demonstrate its understanding of and ability to comply with all applicable Federal, State of Louisiana, Department of Defense, DHA, Army EMS, OSHA, HIPAA, and other regulatory requirements governing contract performance.

Subfactor 2: Personnel Qualifications and Staffing
- Offeror shall provide a staffing plan identifying labor categories, staffing levels, work schedules, and surge staffing capability necessary to meet all Performance Work Statement (PWS) requirements.
- Offeror shall provide an organizational chart identifying key personnel, including the Program Manager, EMS Medical Director, Paramedics, and EMTs.
- Offeror shall provide resumes for all key personnel demonstrating the education, certifications, licensure, qualifications, and experience required to perform the services outlined in the PWS.
- Offeror shall demonstrate its ability to recruit, retain, and sustain qualified personnel while maintaining required staffing levels and minimizing vacancies to ensure uninterrupted mission support.

Subfactor 3: Vehicle and Equipment Maintenance
- Offeror shall provide a comprehensive vehicle and equipment maintenance plan demonstrating how ambulances and medical equipment will be inspected, cleaned, tested, and maintained in accordance with manufacturer recommendations and applicable regulatory requirements.
- Offeror shall demonstrate its capability to maintain continuous operational readiness through preventive maintenance, equipment inspections, inventory control, and timely replacement or repair of vehicles and medical equipment.
- Offeror shall provide procedures for documenting vehicle inspections, equipment maintenance, corrective actions, and quality assurance activities to ensure uninterrupted mission support.
- Offeror shall demonstrate its ability to maintain operational capability during vehicle or equipment failures through contingency planning and resource management in accordance with the Performance Work Statement.

Subfactor 4: Quality Management and Performance Assurance
- Offeror shall provide a Quality Control Plan (QCP) demonstrating how contract performance will be monitored to ensure compliance with all Performance Work Statement (PWS) requirements.
- Offeror shall describe its quality management processes for identifying, documenting, correcting, and preventing recurring performance deficiencies throughout contract performance.
- Offeror shall provide procedures for monitoring personnel qualifications, certifications, credentialing, mandatory training, and regulatory compliance to ensure continuous mission readiness.
- Offeror shall provide a continuous quality improvement approach demonstrating how performance data, customer feedback, and operational trends will be used to improve service delivery and reduce performance risk.

Factor 2. Past Performance: The Offeror shall submit with its proposal evidence of recent and relevant past performance under which YOUR company (the Offeror) provided the same or similar services in the public and/or private sector. Performance must have been by the Offeror (not by a subsidiary or sister company) either as the prime contractor or subcontractor. 

Offerors shall complete and provide the Project Summary Sheet (Attachment 1) on three (3) task orders or stand-alone contracts/purchase orders the Offeror will complete or has completed within the past three (3) years from the closing date of this solicitation that the Offeror considers most relevant in demonstrating YOUR company's (the Offeror's) ability to perform the proposed effort. Past Performance must be with the Department of the Army, other US Government departments or agencies, state and local governments and/or commercial/private sector clients. If the Offeror performed as a subcontractor on the reference project, the Project Summary Sheet shall explain what percentage and type of work that was performed by the Offeror as subcontractor. In the instance that an offeror submits more than three (3) Project Summary Sheets, only the first three (3) will be evaluated. Offerors shall use the attachment provided.

Past Performance Questionnaires (PPQs) (Attachment 2) may be submitted (however not required) for each past performance reference identified on the project summary sheet (Attachment 1).
Contractor Performance Assessment Reports (CPARs) will also be accepted. For projects that have CPARs, Offerors can submit CPARs with Project Summary Sheets in lieu of PPQs.
Offerors shall ensure CPARs are finalized and reference the exact same projects detailed in the
Project Summary Sheet.
If a PPQ or CPARs was not received for any of the past performance references identified on the project summary sheet, the Government may conduct interviews to validate performance/quality information. A past performance reference will not be evaluated if it does not contain valid/current POC contact information, or if the POC provided cannot validate work identified on
the project summary sheet.
The Government reserves the right to use both the information provided in the Offeror's Past Performance submittals and information obtained from other sources available to the Government, to include, but not limited to, the CPARs, Federal Awardee Performance and Integrity Information System (FAPIIS), Electronic Subcontract Reporting System (eSRS), or other databases; interviews with Program Managers, Contracting Officers, and commercial sources and/or non-Government sources, such as any commercial business or customers referenced by the Offeror in any of the past performance information disclosed.

Factor 3. Price: Offeror shall complete the pricing for each of the contract line items (CLINs) on the Pricing Worksheet (Attachment 3).
