Elevator Third-Party Inspection and Testing Services for the Brooklyn VA Medical Center and St. Albans VA Medical Center Description This is a combined synopsis/solicitation for commercial products and commercial services prepared in accordance with the format in Revolutionary FAR Overhaul (RFO) in Federal Acquisition Regulation (FAR) subpart 12.202, Streamlined Procedures for Evaluation and Solicitation for Commercial Products and Commercial Services, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested, and a written solicitation document will not be issued. This solicitation is issued as an RFQ. Services shall include the performance of a Five-Year Safety Test (Category 5) inspection as well as annual and semiannual Safety Tests inspections (Category 1) for elevators and dumbwaiters. All services shall be performed at the following addresses- Address: Brooklyn VA Medical Center 800 Poly Place Brooklyn, NY 11209-7104 St. Albans VA Medical Center 179-00 Linden Boulevard Queens, NY 11424-1468 OBJECTIVE: The Contractor shall inspect five-year safety tests when applicable and annual and semiannual safety test of elevators and dumbwaiters in accordance with all requirements set in the base year and all four option periods. PERFORMANCE PERIOD: The government requires a 12-month base contract period, including four option years Base: 09/07/2026 09/06/2027 Option 1: 09/07/2027 09/06/2028 Option 2: 09/07/2028 09/06/2029 Option 3: 09/07/2029 09/06/2030 Option 4: 09/07/2030 09/06/2031 ANNUAL AND SEMIANNUAL SAFETY TEST: An annual and semiannual safety test will be performed on elevators/dumbwaiters. Safeties shall be subjected to an annual and semiannual running test as indicated per OSHA 1917.116 / ASME A17.1. The slowest operating speed, at no load test required for annual test as indicated as per OSHA 1917.116 / ASME A17.1. The Maintenance Contractor (separate vendor providing testing services) is required to furnish the tools and labor to perform the annual and semiannual no-load tests. Phase-1 Lobby Key Switch and Phase-2 Service check should be accomplished during the annual and semiannual safety check. The Maintenance Contractor shall attach tags after testing as required by ASME A17.1 and A17.2. The Maintenance Contractor shall not take out more than one elevator at a time and coordinate with facility for not impacting patient care or operations of the facility. The Maintenance Contractor provides all required equipment (inspectors do not supply, handle, or move weights). Note: The details above describe the procedures carried out by the Maintenance Contractor, but the requirements related to inspecting the Annual and Semiannual Safety Tests inspection. ANNUAL AND SEMIANNUAL SAFETY TEST INSPECTIONS: Inspection to be performed January or within 30 days of award. Semiannual to be performed in July or six months after the annual inspection. The Contractor will witness and certify the inspection. Inspections are to be made in accordance with the Safety Code for Elevators, ASME A17.1 latest edition. Inspections and documentation shall be in accordance with American Society of Mechanical Engineers A17.1, A17.2, A17.3, A17.4 most recent edition and addenda standard. The COR shall accompany the contractor as an observer during the entire time of each inspection. The Contractor Inspectors do not perform maintenance or tests; they only witness, certify, and report FIVE YEAR SAFETY TEST: A five-year safety test will be performed on elevators and dumbwaiters listed during the base year of the contract within 30 days of award. Safeties shall be subjected to the ASME testing standards for car and counterweights, governors and oil buffers as per OSHA 1917.116 / ASME A17.1. 6167, Report of Test of Elevator Governor and Safety Device which is included in this contract. Test leveling zone and leveling speeds with contract load. Test the door pressure and kinetic energy. Test governors pull through tension. Record the following but not limiting to: capacity, speed empty up and down, speed loaded up and down, safety jaw tripping speed, over-speed governor tripping speed, safety slide on the rails, governor pull through, governor plate speed, car level after test, buffer return time, complete firefighters service check as applicable, and safety rope pull out as applicable. A five-year load test is required. Testing requires test weights. No fifty-five-gallon drums filled with water or 1000 lb weights will be accepted for use on load tests. The Maintenance Contractor (separate vendor providing testing services) is required to furnish weights, tools, and labor to perform the five-year full load test. All work is to be performed in accordance with 5-year inspection test requirements, ASME A17.1 latest edition. During the performance of the five (5) year test requirements, adjacent elevators shall be removed from service in the interest of safety in accordance with ASME A17.2.1. The Maintenance Contractor shall not take out more than one elevator at a time and coordinate with facility for not impacting patient care or operations of the facility. Elevators are to be run at slow speed. The Maintenance Contractor provides all required equipment (inspectors do not supply, handle, or move weights). Note: The details above describe the procedures carried out by the Maintenance Contractor, but the requirements relate to inspecting the Five-Year Safety Test inspection. FIVE YEAR SAFETY TEST INSPECTION: Perform all inspections and tests in strict accordance with ANSI/ASME A17.1 plus A17.2. List code violations observed during tests. The Contractor will witness and certify the inspection. Inspections and documentation shall be in accordance with American Society of Mechanical Engineers A17.1, A17.2, A17.3, A17.4 most recent edition and addenda standard. The COR shall accompany the contractor as an observer during the entire time of each inspection. The Contractor Inspectors do not perform maintenance or tests; they only witness, certify, and report RETESTING AND RE-INSPECTION OF CORRECTIVE ACTIONS: If an elevator or dumbwaiter fails any inspection under this contract, the contractor shall document all cited deficiencies in the inspection report. Once the maintenance contractor completes corrective action, the inspector shall re-inspect the affected unit to verify the deficiencies have been corrected Re-inspections shall be invoiced under the Corrective Action Re-Inspection line item. Re-inspections needed due to the Contractor's own error shall be performed at no additional cost. REPORTS: The Contractor shall submit report(s) upon completion of the inspection. For all items on such forms which are not applicable, the notation "NA" (not applicable) shall be made. Reports will be prepared and submitted- two (2) hard copies and two (2) electronic copies, no later than 15 working days following the inspection to the VA Medical Center Attention: Engineering Service (138), COR as noted at time of award. SCHEDULE OF INSPECTIONS AND WORK HOURS: The annual safety test shall be made during the months indicated on the list of elevators/dumbwaiters for the VA Medical Center. Work required in the performance of this contract shall be performed during the VA Medical Center's regular administrative working hours, which are 8:00 a.m. to 4:30 p.m., Monday through Friday, excluding Federal holidays. The Contractor and Maintenance Contractor shall schedule/coordinate inspection and testing of listed units with COR and Service Company two weeks prior to inspection/testing procedures. Upon arrival to perform inspection all service contract personnel shall check in and out before providing services. Location for checking in and out during normal business hours will be indicated at time of award. Service tickets must be signed and dated, indicating work complete or incomplete. All contractors, without additional expense to the Government, shall obtain all licenses, permits, and insurance required to perform the work in this Contract. QUALIFICATIONS OF INSPECTORS: All Inspectors shall meet the qualification requirements of ASME QEI-1. Inspectors shall be certified by an organization accredited by ASME in accordance with the requirements of ASME QEI-1. Certification must be kept current during the term of the project. When any updates to the certification are completed, a copy must be provided to the CO and COR within 10 calendar days of the update certification. The maintenance contractor shall attach tags after testing as required by ASME A17.1 and A17.2. The maintenance contractor shall identify the date & name of the person of the last Annual Test up to the Five-Year Load Tests on each elevator tag. The maintenance contractor shall provide tools, equipment, specialty items and other items that may be necessary to perform tasks required under this Contract. All contractors are responsible for providing all certifications of training for employees who are performing inspections. The Contractor shall provide a detailed in person brief on each elevator. Information required to conduct a Background Check includes full name, driver s license number and/or social security number, date of birth, and completion of a background check questionnaire. Contractors shall ensure their employees and those of their Subcontractors have the proper credentials allowing them to work in the United States. The Contractor shall not be entitled to any compensation for delay or expenses associated with this requirement and are not excused from proceeding with the Contract. Department of Veterans Affairs regulations always requires Contractor employees to have VA issued Identification Badge and display it while at VA facilities. ID badges shall be issued by VA security office under its rules for issuance. Contractor personnel shall present a neat appearance and be easily recognized as a Contractor employee by wearing distinctive clothing bearing the name of the company and VA issued ID badge. When an employee leaves the Contractor s employment, or the Contract is terminated, the employee s badge shall be returned to COR within 3 working days. The contractor shall be responsible for safeguarding all government equipment, information and property provided for contractor use. At the close of each work period, government facilities, equipment, and materials shall be secured The Contractor shall establish and implement methods of making sure all keys/key cards issued to the Contractor by the Government are not lost or misplaced and are not used by unauthorized persons. NOTE: All references to keys include key cards. No keys issued to the Contractor by the Government shall be duplicated. The Contractor shall develop procedures covering key control that shall be included in the Quality Control Plan. Such procedures shall include turn-in of any issued keys by personnel who no longer require access to locked areas. The Contractor shall immediately report any occurrences of lost or duplicate keys/key cards to the Contracting Officer. In the event keys, other than master keys, are lost or duplicated, the Contractor shall, upon direction of the Contracting Officer, re-key or replace the affected lock or locks; however, the Government, at its option, may replace the affected lock or locks or perform re-keying. When the replacement of locks or re-keying is performed by the Government, the total cost of re-keying or the replacement of the lock or locks shall be deducted from the monthly payment due the Contractor. In the event a master key is lost or duplicated, all locks and keys for that system shall be replaced by the Government and the total cost deducted from the monthly payment due the Contractor. The Contractor shall prohibit the use of Government issued keys/key cards by any persons other than the Contractor s employees. The Contractor shall prohibit the opening of locked areas by Contractor employees to permit entrance of persons other than Contractor employees engaged in the performance of assigned work in those areas, or personnel authorized entrance by the Contracting Officer. The Government has unlimited rights to all documents/material produced under this contract. All documents and materials, to include the source codes of any software, produced under this contract shall be Government owned and are the property of the Government with all rights and privileges of ownership/copyright belonging exclusively to the Government. These documents and materials may not be used or sold by the contractor without written permission from the Contracting Officer. All materials supplied to the Government shall be the sole property of the Government and may not be used for any other purpose. This right does not abrogate any other Government rights. ELEVATOR OUT OF SERVICE: Before removing an elevator from service, notification is required to the following: Engineering Service (Boiler Plant shall be notified after-hours) Brooklyn VA Police Service (After-Hours alternate) or St. Albans VA Police Service (After-Hours alternate) and the COR immediately during normal business hours, either by phone or in person. If work requires more than one day, daily notification of the out of service condition will be made. Notify the COR when placing the elevator back in service. When an elevator is taken out of service, appropriate signage shall be installed at each entry door to notify elevator users of the interruption. Contractor shall provide signage and post in a manner which does not damage paint or other finishes. Upon completion, all signage shall be immediately removed. The COR will approve signage plaques used by the Contractor within 10 days of contract award. Signs shall be no smaller than 6 x4 with 48-point type, in English. Tests, inspections, or maintenance repairs removing an elevator from service shall be scheduled in advance with the CO/COR. A schedule for maintenance shall be submitted within 10 days of contract start. Under no circumstances will any shutdown or outage last longer than five (5) consecutive working days, from initial notification to the Contractor, without prior notification and approval of the CO/COR. For unscheduled elevator outages, the maintenance contractor shall provide a detailed description of the outage and required repairs by written notification to the COR by the following business day. SAFETY: Contractor shall adhere to OSHA, EPA, NFPA, VA Life Safety Codes, and other regulatory requirements. Safety shall be the Contractor s highest priority. The Contractor shall be solely responsible for compliance with (Federal) OSHA regulations. The Contractor shall provide full elevator service, in compliance with the ASME Safety Code requirements, manufacturer's recommendations, Elevator Industry Field Employees Safety Handbook, National Electrical Code, and all other applicable laws, regulations, rules, ordinances, codes, etc. Contractor shall provide a detailed explanation as to how contract work will be implemented and managed to ensure compliance with OSHA Construction Standards, Life Safety issues, and security measures. Contractor shall have a Site Safety and Health Officer (SSHO) assigned to the contract/project to ensure their technicians are meeting OSHA requirements. The SSHO shall be well versed in OSHA requirements and regulations. The SSHO will perform regular safety inspections on the job site to ensure technician compliance and quality of work is at a high level, presenting no risks to staff or patrons. Safety inspections will include fall protection and electrical hazards and end with job safety meetings with the entire crew. The Contractor shall provide the CO/COR copies of the safety inspection reports upon request at no additional cost to the Government. During contract performance, the Contractor shall follow Government s safety policy and standards for safe work practices and take such safety precautions as the VA Safety Officer or designee or the CO/COR may determine to be reasonably necessary to protect the lives and health of occupants of the building. The Contractor shall comply with VA smoking policy. Patient, employee, visitor, and Contractor personnel safety shall always be maintained. The Contractor is responsible for the safety and health of his/her employees. The Contractor (and/or maintenance contractor) shall replace all safety guards, equipment, devices, etc. removed to service or repair the elevator immediately after completion of work or when leaving the job unattended. Contractor is responsible to identify, provide and maintain personal protective equipment (PPE) required to perform the duties outlined in the Contract. In addition, the Contractor is responsible for identifying and providing applicable safety programs required to perform the work i.e., lockout/tag out, confined space entry, universal precautions, etc. Training required for safety programs and proper use of PPE shall be provided by employer, and documentation maintained by the Contractor. Contractor shall be notified of any non-compliance with safety regulations. The Contractor shall, after receipt of such notice, immediately correct the condition and notify the Contracting Officer in writing that the condition has been corrected. If, in the opinion of the CO/COR or the Government s Safety Officer, the condition is life threatening he/she shall instruct the Contractor to stop work until the condition has been corrected. If the Contractor fails or refuses to comply promptly, the Contracting Officer may issue an order halting all or any part of the work. The Contractor bears all costs of stop-work time and correcting safety hazards. Contractor shall immediately report to the CO/COR in a manner and on the forms prescribed by the Government, accidents resulting in injury, trauma, death, hazardous exposures, and occupational disease involving an elevator or its maintenance. POST AWARD CONFERENCE/PERIODIC PROGRESS MEETINGS: The Contractor agrees to attend any post award conference convened by the contracting activity or contract administration office. The contracting officer, Contracting Officers Representative (COR), and other Government personnel, as appropriate, may meet periodically with the contractor to review the contractor's performance. At these meetings the COR/CO will apprise the contractor of how the government views the contractor's performance and the contractor will apprise the Government of problems, if any, being experienced. Appropriate action shall be taken to resolve outstanding issues. These meetings shall be at no additional cost to the government. The following provisions are incorporated into 52.212-2 as an addendum to this solicitation: The following are the decision factors: The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors will be used to evaluate offers; Technical Capability Past Performance Price Evaluation Approach. The Government reserves the right to consider a quotation other than the lowest price that provides additional benefit(s). Quotations may exceed minimum requirements of the solicitation. The Government reserves the right to select a quotation that provides benefit to the Government that exceeds the minimum requirements of the solicitation but is not required to do so. Each response must meet the minimum requirements of the solicitation. The Government is not requesting or accepting alternate quotations. Failure to provide the information requested may result in being found non-responsive. The evaluation will consider the following: Technical Capability: The quotation will be evaluated to the extent to which it can meet and/or exceed the Government s requirements as outlined in the solicitation. The quotation should provide a clear and demonstrative plan and explanation of the organization s ability to perform Five-Year Safety Test (Category 5) as well as annual and semiannual Safety Tests (Category 1) for elevators and dumbwaiters. Past Performance: The past performance evaluation will assess the relative risks associated with a quoter s likelihood of success in fulfilling the solicitation s requirements as indicated by the quoter s record of past performance. The past performance evaluation may be based on the contracting officer s knowledge of and previous experience with the supply or service being acquired; Customer surveys, and past performance questionnaire replies; Contractor Performance Assessment Reporting System (CPARS) at http://www.cpars.gov/; or any other reasonable basis. Price: The Government will evaluate the price by adding the total of all line-item prices, including all options. The Total Evaluated Price will be that sum. Price will be evaluated to determine whether it is fair and reasonable and if it reflects that the offeror understands the work. The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. As a minimum, quotes must include: A technical capability statement that clearly and concisely describes how the offeror will provide the required services without parroting back each section of the statement of work. This should be a maximum 15-page technical capability statement demonstrating the Vendor s technical capability to meet all the requirements of the statement of work. The technical capability statement shall: a) Provide a written plan which details the approach to accomplishing all the requirements of the Statement of Work within the stated time frames while ensuring compliance with OSHA Construction Standards, Life Safety issues, and security measures b) Provide a sample report and schedule describing the planned service c) Provide training certificates/certifications: In accordance with ASME A17.1, ASME A17.2, and ASME A17.3. The contractor shall also provide a copy of their QEI certification and their National Elevator Industry Education Program (NEIEP) (for their certified mechanics) d) Provide resumes for each personnel demonstrating at least 10 (ten) years of recent experience in the operations and maintenance of the kind of elevators to be maintained under this requirement e) Provide resumes for reach Certified Elevator Mechanic(s) with 10 to 15 years experience on category 5 and category 1 testing and inspection on elevators and dumbwaiters. f) Evidence that technicians have experience working on similar equipment and projects at similar facilities (federal, healthcare, etc.) by providing three contract numbers, dates, and descriptions of previous work performed 2. Terms of any express warranty; 3. Price and any discount terms. Price should be for all services detailed in the Price/Cost Schedule. The pricing must be broken down in a manner that allows for price reasonableness to be determined.
