Homeless to Housing Request for Proposals

The State of Connecticut Department of Mental Health and Addiction Services (DMHAS) issued solicitation SWS-HOUSING-H2H to establish programs that assist individuals experiencing sheltered or unsheltered homelessness in finding and sustaining stable, permanent housing. Utilizing a services follows the person model, the program requires the implementation of interdisciplinary service teams to provide outreach, connect individuals to the Coordinated Access Network (CAN), and offer long-term tenancy support. Key performance goals include ensuring at least 90 percent of consumers have immediate stabilization plans and increasing consumer income through employment or SSI/SSDI. The total available funding is 22,770,513.00 dollars annually, with proposals requiring an itemized three-year budget and a minimum salary of 100,000 dollars per full-time equivalent employee. To qualify, proposers must demonstrate at least five years of experience in direct service delivery and collaboration with housing and mental health networks. Proposals are evaluated on a weighted scale, with 35 points dedicated to service expectations, including canvassing schedules and the use of the Housing Assessment and Acuity Index tool. While the submission deadline was extended to December 10, 2026, Addendum 4 subsequently announced the withdrawal of the solicitation due to legal and financial uncertainties regarding the federal Notice of Funding Opportunity from HUD.
