Diabetes causes illness and death in the United States. It leads to high health care costs. People who use Federally Qualified Health Centers (FQHCs) and other safety-net clinics face greater risks. Many community-clinical linkage (CCL) strategies show benefits when used alone. We know less about how several strategies can be combined and coordinated. Using multiple strategies together may improve diabetes prevention and management in primary care.

This NOFO aims to find what helps or blocks the use of coordinated CCL strategies. We will identify factors that affect adoption, integration, and long-term success. Researchers will study real-world FHQC clinics. Practical evidence will guide best practices for diabetes prevention and management.
