This contract, spanning from June 1, 2026, to May 31, 2029, focuses on integrated, patient-centered care in collaboration with CSC’s interdisciplinary health care teams, addressing infectious diseases including viral hepatitis (B and C), HIV, and tuberculosis among federally incarcerated patients. The scope requires the physician to provide timely, clinically independent care either in-person or via approved remote clinics, adhering to the Person Health Care Home model that emphasizes coordinated interdisciplinary care plans incorporating patient input.
