Full Description
The purpose of this amendment is to make changes to the solicitation. The revisions are marked as 001.
- Extend the submission due date of offers from June 15, 2026, no later than 3:00 PM to June 17, 2026, no later than 3:00 PM (Philippine Local Time). Please refer to the Continuation to SF-18, Page 4, reflecting the changes.
- Under the Continuation to SF-18, Page 3, reflecting the requirement for the deadline for submission of questions.
- Annex D and Annex E — Health Insurance Utilization Reports are hereby added to the solicitation package as additional reference documents.
- All other Terms and Conditions remain unchanged and in full force and effect.
