The purpose of this amendment is to make changes to the solicitation. The revisions are marked as 001.


  1. Extend the submission due date of offers from June 15, 2026, no later than 3:00 PM to June 17, 2026, no later than 3:00 PM (Philippine Local Time). Please refer to the Continuation to SF-18, Page 4, reflecting the changes.
  2. Under the Continuation to SF-18, Page 3, reflecting the requirement for the deadline for submission of questions.
  3. Annex D and Annex E — Health Insurance Utilization Reports are hereby added to the solicitation package as additional reference documents.
  4. All other Terms and Conditions remain unchanged and in full force and effect.
