V-53 Loading Dock Leveler at Door S1 is inoperable. While trying to utilize the leveler nothing appears to happen. Request for the leveler to be repaired/replaced as it is a safety hazard while loading/unloading vehicles to dock. There is an approximate 8”-12” gap between vehicle and dock. GWSRP uses hand trucks and motorized fork vehicles to unload material from vehicles. This gap has caused the front wheels to come off the loading dock and hang in the air. Forks have landed on the vehicle bed, which prevented further damage or injury. If not repair damage to equipment or injury to personnel if forks do not grab truck during loading/unloading.
