The Department of Veterans Affairs, through its Program Contracting Activity Central, intends to sole source a contract for customized survey services focused on High Alert Medication Safety Self-Assessment as a proactive risk assessment tool. The contract aims to monitor and improve the use of high alert medications across all VA sites using an established web-based application developed by the Institute for Safe Medication Practices (ISMP), a recognized Patient Safety Organization. The tool, previously utilized by about 100 sites in 2018, facilitates tracking and trending of medication safety improvements and will now be expanded to include all VA facilities nationwide. The contractor will be responsible for administering the survey, compiling data, granting VA sites access to the web application, and assisting with technical support. Data collection will include identifying survey responses by facility and regional VISN for comprehensive analysis. The government is inviting any qualified sources to submit capability statements or proposals by June 4, 2021, although the VA reserves the right to proceed with sole sourcing based on responses received. Respondents must provide detailed company information, including contact details, Cage code, and Dun & Bradstreet number, and direct submissions to the designated VA contact via email. This effort supports the VA’s ongoing commitment to patient safety in medication management.
