Request for Qualifications (RFQ) Environmental (Home) Modifications,

The Oregon Department of Human Services, Office of Aging and People with Disabilities, is seeking qualified construction contractors to perform environmental home modifications to improve the health, safety, and independence of consumers. The scope of work includes the installation of wheelchair ramps, ceiling tracks, stair lifts, and residential elevators, as well as remodels of kitchens, bathrooms, and bedrooms. This is a two-stage procurement process where qualified applicants are first placed on a master list of approved contractors. Once on this list, contractors are invited to submit lump sum bids for individual projects based on their specified service capabilities and geographic coverage. Awards for specific projects are generally granted to the lowest responsive bidder who can meet the required start date. To qualify for the master list, contractors must be actively registered and unsuspended with the Oregon Construction Contractors Board, possess an active Medicaid provider number, and be registered with the Oregon Secretary of State. Applicants must submit a comprehensive packet including an application certification form, a responsibility inquiry, and a certified disadvantaged business outreach plan. Mandatory requirements include maintaining public liability, personal injury, and property damage insurance per ORS 701.073 and complying with local building codes. Final project acceptance is contingent upon a site inspection, consumer satisfaction, and the provision of photographic or video documentation along with a final inspection report from the building inspector.
