Proof of active business status with the California Secretary of State is required, as are certificates for Workers’ Compensation, Cyber Liability, Automobile Liability (minimum $1,000,000 per occurrence, including non-owned auto), and Professional Liability Insurance, with exceptions allowed only if valid justifications are provided and certificates submitted prior to contract execution. Proposals must include a detailed narrative addressing Understanding and Approach (15%), Corporate Qualifications and References (35%), and Project Team Qualifications and Resumes (20%), totaling 70% of the technical score, and a cost proposal based on the Exhibit B, Attachment 1 - Cost Worksheet accounting for fully loaded hourly rates inclusive of fringe, overhead
