Rural Health Transformation Program - Adoption of a Statewide Closed Loop Referral System

Vermont has secured a $195 million grant in the first year of a five-year Rural Health Transformation initiative aimed at enhancing rural health networks, technology, workforce capacity, and overall care delivery efficiency. This specific contract seeks a qualified third-party organization to manage and implement a statewide digital closed-loop referral system designed to improve collaboration among healthcare, mental health, substance use disorder, and social service providers. The platform will facilitate real-time data sharing and tracking of referral outcomes to better connect patients with essential community supports such as housing, food assistance, and transportation, ultimately reducing service duplication and promoting cost-effective, seamless, person-centered care throughout Vermont’s health and social service systems. The Department of Economic Development, Vermont, has issued this solicitation (RHT_AHS_007_FY26) with a response deadline of June 26, 2026, inviting organizations to serve as the program manager and lead implementer of this critical Shared Services project under the Rural Health Transformation Program.
